摘　　要：目的通过CT肺动脉造影(CTPA)的产出率和指南依从率(CGR)研究,了解急性肺栓塞诊断水平.方法回顾性分析进行CTPA检查的666例患者,计算CTPA总产出率、3年各自产出率及Wells评分各分级产出率,并统计组间的差异性.应用Wells评分和D-二聚体检测评估CGR.结果CTPA总产出率为31.5%.3年各自的产出率为29.7%、37.3%、27.6%,3年间的产出率有统计学差异(P<0.05).根据Wells评分分为低度可能、中度可能、高度可能,产出率分别为8.0%、46.7%、86.4%,3组间产出率有统计学差异(P<0.05).CGR为52.1%.结论CTPA产出率较高,CGR较低,存在不合理应用CTPA检查的现象.Objective To assess the yield of CT pulmonary angiograpby(CTPA)and compliance guidelines rate,to know the diagnostic level of acute pulmonary embolism(PE).Methods 666 cases underwent CTPA were analyzed retrospectively.The yield of CTPA was calculated including overall and three years rate and grades of Wells score,statistical differences were analyzed among different groups.The Wells score and D-dimer results were used to evaluate the CGR.Results The overall yield of CTPA in our study was 31.5%.The three.years yield of CTPA were 29.7%,37.3%and 27.6%respectively with statistical differences(P<0.05).The Wells score was used to.evaluate the clinical probability of PE as low。moderate and high grades,with the yield of CTPA of 8.0%,46.7%and 86.4%,which had stistical dfferences(P<0.05).The CGR was 52.1%.Concluson The yield of CTPA is high,the CGR is low.There is unreasonable usage of CTPA.
