芪参益气滴丸对慢性肺源性心脏病病人和肽素及细胞因子的影响

摘　　要：目的研究芪参益气滴丸对慢性肺源性心脏病病人和肽素及细胞因子的影响。方法选择2016年8月—2018年10月收治的慢性肺源性心脏病病人192例,按随机数字表法分为治疗组与对照组,每组96例。对照组给予氧疗、原发病治疗、强心、利尿等;治疗组在常规治疗基础上加用芪参益气滴丸口服,每次0.5 g,每日3次,连续治疗28 d。比较两组6 min步行距离(6-MWT)、左心室射血分数(LVEF)及血清白细胞介素-4(IL-4)、白细胞介素-6(IL-6)、白细胞介素-10(IL-10)、白细胞介素-12(IL-12)、肿瘤坏死因子-α(TNF-α)、γ-干扰素(IFN-γ)、N末端B型利钠肽原(NT-proBNP)、和肽素水平。结果治疗前,两组NT-proBNP、和肽素水平及6-MWT、LVEF比较,差异均无统计学意义(P>0.05)。治疗28 d后,两组NT-proBNP、和肽素水平降低,6-MWT、LVEF升高,治疗组较对照组变化更明显,差异有统计学意义(P<0.05);治疗前两组IL-4、IL-6、IL-10、IFN-γ、TNF-α、IL-12含量比较,差异无统计学意义(P>0.05);治疗后两组IL-4、IL-6、IL-10含量降低,IFN-γ、TNF-α、IL-12含量升高,治疗组较对照组变化明显,差异均有统计学意义(P<0.05)。治疗前慢性肺源性心脏病病人血清NT-proBNP与IFN-γ/IL-4比值呈负相关(r=-0.892,P=0.001)。结论芪参益气滴丸能降低NT-proBNP、和肽素水平,可通过调整细胞因子水平,调节细胞应答平衡,缓解临床症状。Objective To study the effects of Qishen Yiqi dropping pills on the levels of copeptin and cytokines in patients with chronic pulmonary heart disease.Methods One hundred and ninety-two patients with chronic pulmonary heart disease were randomly divided into treatment group(n=96)and control group(n=96).The patients in control group were treated with oxygen therapy,primary disease therapy,strengthening heart,and diuretic therapy,and the patients in treatment group were treated with Qishen Yiqi dropping pills for 28 days,0.5 g a time,3 times a day.The 6-minute walking distance(6-MWT),left ventricular ejection fraction(LVEF),the levels of IL-4,IL-6,IL-10,IL-12,TNF-α,IFN-γ,NT-proBNP,and copeptin were measured before and after treatment.Results Before treatment,there was no significant difference between two groups in NT-proBNP,copeptin levels,and 6-MWT,LVEF(P>0.05).After 28 days of treatment,the concentration of NT-proBNP and copeptin in two groups decreased and 6-MWT,LVEF increased,the data in treatment group changed more obviously than those in control group(P<0.05).Before treatment,there was no significant difference in the content of IL-4,IL-6,IL-10,IFN-γ,TNF-α,IL-12(P>0.05).between two groups.After treatment,the content of IL-4,IL-6,and IL-10 in two groups decreased,and the levels of IFN-γ,TNF-α,and IL-12 in two groups were increased,the data in treatment group were changed more obviously than those in control group(P<0.05).Before treatment,there was a negative correlation between serum NT-proBNP and IFN-γ/IL-4 ratio in patients with chronic pulmonary heart disease(r=-0.892,P=0.001).Conclusion Qishen Yiqi dropping pills can reduce the levels of NT-proBNP and copeptin,adjust cytokine level,regulate the balance of cell response,relieve clinical symptoms.
