叶天士治疗湿热型脾胃病的用药规律研究

摘　　要：目的通过统计分析叶天士治疗湿热型脾胃病医案中所用处方,总结其用药规律。方法收集叶天士治疗湿热型脾胃病的医案57例,将处方录入中医传承辅助平台系统,进行相关数据分析。结果收集57首处方,共计93味中药,其中高频使用药物为黄连、茯苓、厚朴、半夏、黄芩等;集中使用温药、寒药,多用苦、辛、甘味药;药物多归于脾、胃、肺经;高频(频次≥11)药物组合12组;推演出核心组合2个、新处方2首。结论叶天士治疗湿热型脾胃病着重从中焦太阴阳明入手,亦重视从肺论治的思想;用药上祛湿与调畅气机并重,辅以清热,注重固本和中,黄连、半夏、枳实、茯苓为其核心药物,为临床治疗湿热型脾胃病提供借鉴思路。Objective To summarize and study the medication law of YE Tianshi through the statistics and analysis of the drugs used in the treatment of spleen and stomach diseases with dampness-heat type.Methods Fifty-seven cases of spleen and stomach diseases with dampness-heat typetreated by YE Tianshi were collected,and the prescriptions were recorded into the TCM Inheritance Platform System for data analysis.Results A total of 57 prescriptions were collected and 93 kinds of traditional Chinese medicine were used.The drugs with high using frequency were Huanglian(Coptis chinensis Franch.),Fuling(Poria cocos),Houpo(Magnolia officinalis),Banxia(Pinellia ternata),Huangqin(Scutellaria baicalensis).Most are warm,cold,bit,pungent and sweet drugs.Drugs mostly attributed to the spleen,stomach,lung meridians.There were 12 groups of drug combinations with high using frequencyand performance 2 core combinations and 2 new prescriptions were obtained.Conclusion The treatment of spleen and stomach diseases of dampness-heat type by YE Tianshi should focus on Yin and Yang of middle-Jiao and the lung.The combination of clearing damp and regulating Qi was emphasized,supplemented by clearing heat.Huanglian(rhizoma coptidis),Banxia(pinaxia),Zhishi(immature bitter orange) and Fuling(poria cocos) were as its core drugs,providing reference ideas for clinical treatment of spleen and stomach diseases of dampness-heat type.
