李帆冰运用“虚补”思想论治膝骨性关节炎经验

摘　　要：回顾性分析并总结李帆冰教授中医对膝骨性关节炎(KOA)认识及临床总体治疗方案。李帆冰教授长期从事膝骨性关节炎的预防与治疗研究,拥有一套独特的理解及治疗,形成了未病先防、中药措施干预调幅的基本思想,对膝骨性关节炎深入研究及观察,同时提高整体诊疗经验,将膝痹、普通痹证异同点相应提出,后以虚证为出入点分析膝骨性关节炎发生基础,形成了中医辨证、辨病、辨体三位一体相结合方法,文章中通过对膝骨性关节炎发病机制分析、辨证分型及其后续中医药治疗状况加以分析,以李帆冰教授思想"虚证"为基准,对肝肾亏虚,气血不足,寒湿痹阻,湿热流注为主证,并治以"以气为主、以血为先、痰瘀同治、内外兼顾、调治结合"的学术思想,对膝骨性关节炎加以诊断、治疗,对膝骨性关节炎有着较深造诣及独到见解。从而达到维持局部血脉通畅,将气机调顺、阴阳调和目的发挥出来,达到预防膝骨关节炎疾病目的。Professor LI Fanbing’s understanding of knee osteoarthritis(KOA) and the overall clinical treatment plan were retrospectively analyzed and summarized.Professor LI Fanbing has been engaged in the prevention and treatment of knee osteoarthritis for a long time.He has a unique set of understanding and treatment.He has formed the basic idea of prevention before illness and intervention of amplitude modulation by traditional Chinese medicine.He has conducted in-depth research and observation on knee osteoarthritis,improved the overall diagnosis and treatment experience,proposed the similarities and differences between knee arthralgia and common arthralgia correspondingly,and then analyzed the occurrence basis of knee osteoarthritis with deficiency disease as the entry and exit point,forming a trinity combination method of TCM syndrome differentiation,disease differentiation and body defense.In this article,through the analysis of the pathogenesis,syndrome differentiation and classification of knee osteoarthritis and its subsequent treatment with traditional Chinese medicine,taking professor LI Fanbing’s thought of"deficiency syndrome"as the benchmark,the diagnosis and treatment of knee osteoarthritis are based on the main syndromes of liver and kidney deficiency,Qi and blood deficiency,cold and dampness arthralgia,and damp-heat flow,and the academic thought of "Qi-based,blood-based,phlegm and blood stasis treatment,internal and external consideration,and combination of regulation and treatment",which has deep attainments and unique insights on knee osteoarthritis so as to achieve that aim of maintain local blood vessels unobstructed,regulate Qi activity and harmonizing Yin and Yang,and preventing knee osteoarthritis diseases.
