摘　　要：目的探究益肾活血降浊法治疗慢性肾衰竭及其对心血管的保护作用机制。方法选取2018年9月—2019年3月该院收治的慢性肾衰竭患者64例,采用随机数字表法分为对照组和治疗组,各32例。其中对照组予以常规西医治疗;治疗组在对照组的基础上加以益肾活血降浊法为原则的中药汤剂,1剂/d,两组患者均治疗12周。比较两组患者血肌酐(Scr)、内生肌酐清除率(Ccr)、尿素氮(BUN)等肾功能改善情况、血清CK-MB、cTnI及BNP、中医症状积分及临床疗效。结果经治疗,两组患者Scr、Ccr、BUN水平均有所改善,且治疗组患者Scr、BUN含量降低均更为显著(P<0.05);两组患者CK-MB、cTnI及BN水平均较治疗前有所降低,且治疗组降低幅度较对照组显著(P<0.05);两组患者中医症状积分均较治疗前有所降低,且治疗组患者降低程度较对照组显著(P<0.05);治疗组患者临床总有效率为90.62%(29/32),显著优于对照组患者的68.75%(22/32),差异具有统计学意义(P<0.05)。结论益肾活血降浊法为指导原则的中药煎剂在治疗慢性肾衰竭方面取得较为满意的临床疗效,具有益肾降浊、清热活血之功效,改善临床症状及肾功能相关指标,保护心血管功能,延缓疾病发展进程,提高患者生活质量,值得临床推广应用。Objective The purpose of this study was to investigate tonifying kidney and activating blood circulation and descending the turbid method treating chronic renal failure and its protective effect on cardiovascular diseases.Methods Sixty-four chronic renal failure patients admitted to our hospital from September 2018 to March 2019 were selected and randomly divided into treatment group and control group with 32 cases in each group according to the table of random number.The control group was administered with conventional western treatment,while the treatment group was administered with TCM decoction on the basis of the control group.The decoction,which is based on the principle of tonifying kidney and activating blood circulation and descending the turbid method,was given one dose per day.The patients in both groups were treated for 12 weeks.We compared improvements of renal function indexes such as Scr,Ccr,BUN,serum CK-MB,cTnI and BNP levels,TCM symptom scores and clinical efficacy.Results After treatment,the decrease of levels of Scr,Ccr and BUN,and the improvement of level of BUN in the treatment group were more significant than those in the control group(P<0.05).The levels of CK-MB,cTnI and BNP in both groups were lower than those before treatment,and the reduction in the treatment group was more significant than that in the control group(P< 0.05).The TCM symptom scores in both groups were lower than those before treatment,and the decrease in the treatment group was more significant than that in the control group(P<0.05).The total clinical effective rate of the treatment group and control group was 90.62%(29/32) and 68.75%(22/32),respectively.The effective rate of the treatment group was significantly better than that of the control group,and the difference was statistically significant(P<0.05).Conclusion TCM decoction based on the tonifying kidney and activating blood circulation and descending the turbid method has achieved satisfactory clinical effects in the treatment of chronic renal failure.It has the ef
