摘 要:目的观察并探讨全蝎软膏对兔耳增生性瘢痕的治疗效果及组织中Smad 7蛋白表达的影响。方法将16只新西兰大白兔随机分为空白对照组、模型组、积雪苷霜软膏组和全蝎软膏组,每组4只。除空白对照组外,其他3组分别进行兔耳增生性瘢痕造模术,术后立即使用积雪苷霜软膏和全蝎软膏对积雪苷霜软膏组和全蝎软膏组进行连续用药治疗,对照组和模型组用磷酸缓冲盐溶液(phosphate buffer saline,PBS)处理,每日2次并拍照记录;分别在治疗后的第35天取模型组、治疗组和空白组动物相应耳组织进行病理、酶联免疫法(enzyme linked immunosorbent assay,ELISA)和Smad 7蛋白检测。结果与空白对照组相比,模型组兔耳术后形成增生性瘢痕,全蝎软膏组治疗后无瘢痕出现,而积雪苷霜软膏组出现轻微瘢痕。苏木精-伊红染色(hematoxylin-eosin staining,HE)结果显示,经积雪苷霜软膏和全蝎软膏治疗后组织纤维化程度均减轻。ELISA检测结果表明,全蝎软膏组与积雪苷霜软膏组相比,Ⅰ型胶原蛋白(Col Ⅰ)、Ⅲ型胶原蛋白(ColⅢ)及肿瘤坏死因子α(tumor necrosis factor alpha,TNF-α)的表达量均显著降低(P<0.05)。免疫印迹结果显示模型组组织中Smad 7蛋白水平显著降低,而全蝎软膏和积雪苷霜软膏治疗后均可显著升高Smad 7蛋白表达(P<0.05),但全蝎软膏治疗效果更明显。结论与积雪苷霜软膏相比,全蝎软膏对增生性瘢痕的疗效更显著,可有效减少组织内胶原蛋白的沉积并通过上调Smad 7蛋白的表达调控转化生长因子-β(transforming growth factor-β,TGF-β)细胞通路而抑制增生性瘢痕的形成。Objective To observe and investigate the therapeutic effect of scorpion ointment on rabbit’s hypertrophic scars and the effect of Smad 7 protein expression in tissues. Method The sixteen New Zealand white rabbits were randomly divided into control group,model group,asiaticoside ointment group and scorpion ointment group. Except for control group,the other three groups were treated with rabbit’s hypertrophic scars model. The asiaticoside ointment and scorpion ointment were used immediately to treat the asiaticoside ointment group and scorpion ointment group continuously,while the control group was not treated and the model group was treated with phosphate buffer saline(PBS)twice a day and photographed. On the 35th day,the corresponding ear tissues of model group,treatment group and blank group were taken for pathological examination,enzyme-linked immunosorbent assay(ELISA)and Smad 7 protein detection. Results Compared with the control group,hypertrophic scars were formed in the model group after surgery and the corpion ointment group had no scars after treatment,while asiaticoside ointment group had slight scars. The results of hematoxylin-eosin staining(HE)showed that the degree of tissue fibrosis was alleviated after the treatment of asiaticoside and scorpion ointment. ELISA results showed that the expression of Col Ⅰ,Col Ⅲ and tumor necrosis factor-α(TNF-α)in scorpion ointment group was significantly lower than that in asiaticoside group(P<0.05). The Western blot results showed that the level of Smad 7 protein in the model group was significantly decreased,while the expression of Smad 7 protein was significantly increased after treatment with scorpion ointment and asiaticoside cream ointment(P<0.05)and the effect of scorpion ointment was better. Conclusion Compared with asiaticoside ointment,scorpion ointment is more effective in treating hypertrophic scars,which can effectively reduce the deposition of collagen in tissues and regulate the formation of hypertrophic scars by increasing the expression o
