香砂六君合半夏泻心汤对糖尿病胃轻瘫大鼠胃动力影响

摘　　要：目的探讨香砂六君合半夏泻心汤对糖尿病胃轻瘫大鼠胃动力的调控作用及其作用机制。方法将50只SD大鼠随机分为正常组10只和造模组40只,造模组采取一次性腹腔注射80 mg/kg链脲佐菌素联合高脂高糖不规则饮食以建立糖尿病胃轻瘫模型,造模成功的大鼠随机分为造模组、莫沙必利组以及香砂六君合半夏泻心汤组,每组10只,连续灌胃给药8周。每2周测定各组大鼠体质量和随机血糖,给药8周后测定各组大鼠胃排空率和小肠推进率,Western blot法检测胃窦组织c-kit蛋白表达,免疫组化法检测胃窦组织Cajal间质细胞(ICC)变化和α-SMA蛋白表达,Masson染色法观察胃平滑肌纤维化情况,ELISA法检测血清中胃泌素(GAS)、胃动素(MTL)、P物质(SP)、生长抑素(SS)、胆囊收缩素(CCK)含量。结果给药8周后,与造模组比较,香砂六君合半夏泻心汤组体质量和胃排空率升高,胃窦组织中ICC数量增加,c-kit蛋白表达升高而α-SMA蛋白表达降低,纤维化程度明显减轻,血清中GAS和MTL含量降低,SP和CCK含量升高,差异均有统计学意义(P<0.05或P<0.01)。结论香砂六君合半夏泻心汤对糖尿病胃轻瘫大鼠有一定的胃排空促进作用,其作用机制可能与调节胃肠激素紊乱和改善胃平滑肌病变等相关。Objective To explore the potential mechanism of Xiangsha Liujun and Banxia Xiexin Decoction treatment on gastric motility of diabetic gastroparesis rats. Methods Sixty SD rats were randomly divided into normal group(n=10)and model group(n=50). The model group rats were intraperitoneally injected of 80 mg/kg streptozotocin and fed with high-fat and high-sugar irregular diet to establish diabetic gastroparesis model. Rats with successfully modeling were randomly divided into model group(n=10),Mosapride treatment group(n=10)and Xiangsha Liujun and Banxia Xiexin Decoction treatment group(n=10). Continuous intragastric administration for 8 weeks,weight and blood glucose of each group were measured every 2 weeks. After 8 weeks,the gastric emptying rate and small intestinal propulsion rate were measured. The expression of c-kit in gastric antrum smooth muscle tissue was detected by Western blotting,and the gastric antrum smooth muscle tissue Cajal interstitial cells,α-SMA expression detected by immunohistochemistry,Masson staining was used to observe the fibrosis of gastric smooth muscle,ELISA method was used to detect serum motilin(MTL),gastrin(GAS),substance P(SP),somatostatin(SS)and cholecystokinin(CCK). Results After 8 weeks of administration,compared with the model group,the weight and gastric emptying rate in Xiangsha Liujun and Banxia Xiexin Decoction treatment group were significantly improved. The number of ICC in gastric antrum increased,the expression of c-kit protein was increased and the expression of α-SMA protein was decreased,and the degree of fibrosis in gastric antrum was significantly reduced. The content of MTL and GAS in serum decreased,and the content of SP and CCK increased. The difference was statistically significant(P<0.05,P<0.01). Conclusion Xiangsha Liujun and Banxia Xiexin Decoction has a certain promoting effect on gastric emptying in DGP rats,and its mechanism may be related to regulating gastrointestinal hormone disorders and improving gastric smooth muscle lesions.
