知柏地黄汤加减联合伐昔洛韦对女性生殖器疱疹患者机体免疫功能调节及血清TNF-α、IL-4、IL-6水平变化的影响

摘　　要：目的:观察知柏地黄汤加减联合伐昔洛韦对女性生殖器疱疹患者机体免疫功能调节及血清肿瘤坏死因子-α(TNF-α)、白细胞介素-4(IL-4)、IL-6水平变化的影响。方法:选取我院65例女性生殖器疱疹患者,按照随机数字表法分组,对照组32例予以伐昔洛韦治疗,观察组33例予以伐昔洛韦+知柏地黄汤加减治疗,观察比较两组临床疗效及细胞免疫(CD8^+、CD4^+)、体液免疫[免疫球蛋白M(IgM)、IgG]、血清TNF-α、IL-4、IL-6水平变化情况,并统计毒副反应发生率及生活质量(DLQI)评分。结果:观察组治疗总有效率为90.91%(30/33),高于对照组65.63%(21/32),差异有统计学意义(P<0.05);用药3个疗程后观察组CD4^+及IgM、IgG水平均高于对照组,CD8^+低于对照组,差异有统计学意义(P<0.05);用药3个疗程后观察组血清TNF-α、IL-6及IL-4水平均小于对照组,差异有统计学意义(P<0.05);观察组毒副反应发生率为15.15%(5/33),对照组为9.38%(3/32),组间对比差异无统计学意义(P>0.05);观察组用药3个疗程后DLQI评分较对照组低,差异有统计学意义(P<0.05)。结论:知柏地黄汤加减联合伐昔洛韦治疗女性生殖器疱疹效果显著,安全性高,可调节机体免疫,改善患者炎性因子水平及生活质量。Objective:To investigate the effect of Modified Zhibai Dihuang Decoction combined with valaciclovir on the immune function regulation and serum tumor necrosis factor-α(TNF-α),interleukin-4(IL-4),IL-6 for patients with female genital herpes.Methods:Selected 65patients with female genital herpes in our hospital were randomly divided into controlled group(n=32)and observation group(n=33),the controlled group was treated with valacyclovir,the observation group was treated with Valacyclovir+Modified Zhibai Dihuang Decoction,clinical efficacy and cellular immunity(CD8+,CD4+),humoral immunity[immunoglobulin M(IgM),IgG],serum TNF-α,IL-4 and IL-6levels were observed and compared between the two groups,and statistically analyzed toxicity incidence and quality of life(DLQI)score.Results:The total effective rate in the observation group 90.91%(30/33)was higher than the controlled group 65.63%(21/32),the difference was statistically significant(P<0.05).The CD4+,IgM and IgG levels in the observation group were higher than those in the controlled group after 3courses of treatment,and CD8+was lower than the controlled group,the difference was statistically significant(P<0.05).The serum TNF-α,IL-6 and IL-4levels in the observation group were significantly lower than those in the controlled group after 3courses of treatment(P<0.05).There was no significant difference in the incidence of side effects between the observation group 15.15%(5/33)and the controlled group 9.38%(3/32)(P>0.05).The DLQI score of the observation group was lower than that of the controlled group after 3 courses of treatment(P<0.05).Conclusion:Using Modified Zhibai Dihuang Decoction combined with Valaciclovir in the treatment of female genital herpes gets a significant effect,high safety,can regulate the immune system,improve the level of inflammatory factors and quality of life.
