养血祛风止痒汤坐浴配合卤米松软膏治疗肛周湿疹临床观察

摘　　要：目的:观察养血祛风止痒汤坐浴配合卤米松软膏治疗肛周湿疹的临床效果。方法:选取我院2017年1月~2018年12月收治的140例肛周湿疹患者为研究对象,采用随机数字表法分为治疗组(70例)和对照组(70例),对照组予以卤米松软膏治疗,治疗组予以卤米松软膏+养血祛风止痒汤坐浴治疗,两组均治疗2周。比较两组的临床疗效、治疗前后的中医症候积分、炎性因子[白介素-2(IL-2)、白介素-4(IL-4)及干扰素-γ(INF-γ)水平],采用欧洲AD研究组(ETFAD)提出的评分标准(SCORAD)及皮肤病生活质量指数量表(DLQI)评估两组患者治疗前后的症状及生活质量。结果:治疗组的治疗总有效率(97.14%)显著高于对照组(82.86%)(P<0.05);治疗组的各项中医症候积分显著低于对照组(P<0.05);治疗组的IL-2、INF-γ显著高于对照组(P<0.05),IL-4显著低于对照组(P<0.05);治疗组的SCORAD、DLQI评分均显著低于对照组(P<0.05)。结论:肛周湿疹运用养血祛风止痒汤坐浴联合卤米松软膏治疗效果显著,明显改善瘙痒、湿疹、红肿等临床症状,提高生活质量。Objective:To explore the clinical effect of applying Yangxue Qufeng Zhiyang Decoction hip bath and Halometasone Ointment in the treatment of perianal eczema.Methods:140patients with perianal eczema admitted to our hospital from January 2017to December 2018were selected as the research objects.They were divided into treatment group(70cases)and controlled group(70cases)by using the method of random number table.The controlled group was treated with Halometasone Ointment,and the treatment group was treated with Halometasone Ointment+Yangxue Qufeng Zhiyang Decoction hip bath for two weeks.The clinical efficacy,TCM symptom score,inflammatory factors(IL-2,IL-4 and IFN-γ)before and after treatment were compared between the two groups.The symptom and quality of life of the two groups were evaluated by SCORAD and DLQI.Results:The total effective rate of the treatment group(97.14%)was significantly higher than that of the controlled group(82.86%)(P<0.05).The TCM symptom scores of the treatment group were significantly lower than that of the controlled group(P<0.05).The IL-2,INF-γof the treatment group were significantly higher than that of the controlled group(P<0.05),the IL-4 was significantly lower than that of the controlled group(P<0.05).The SCORAD and DLQI scores of the treatment group were significantly lower than that of the controlled group(P<0.05).Conclusion:The treatment of perianal eczema with Yangxue Qufeng Zhiyang Decoction hip bath and Halometasone Ointment is effective,which can improve the clinical symptoms such as pruritus,eczema and redness,and improve the quality of life.
