摘 要:目的分析慢性阻塞性肺疾病患者拔管后序贯经鼻高流量氧疗的疗效。方法选取2018年12月至2019年1月本院收治的慢性阻塞性肺疾病患者90例,根据入院时间分为研究组与对照组,各45例。对照组拔管后选择无创机械通气法,研究组拔管后选择序贯经鼻流量氧疗法,比较两组患者治疗效果、治疗前后肺功能指标、氧合指数、不良反应、ICU停留时间及病死率。结果研究组治疗效果优于对照组(P<0.05)。治疗前,两组患者FEV1、FEV1%、氧合参数比较差异无统计学意义;治疗后,研究组患者FEV1、FEV1%、氧合参数均高于对照组(P<0.05)。研究组患者不良反应发生率为11.11%,明显低于对照组的33.33%(P<0.05)。研究组ICU停留时间(12.65±0.44)h、病死率(2.22%)均短于对照组[(15.89±0.88)h、26.67%](P<0.05)。结论慢性阻塞性肺疾病患者拔管后序贯经鼻高流量氧疗治疗效果显著,可明显改善肺功能指标、氧合参数,防止再插管、误吸、压疮发生,且安全性高,ICU监护停留时间短,病死率低,值得临床推广运用。Objective To analyze the efficacy of sequential nasal high-flow oxygen therapy in patients with chronic obstructive pulmonary disease after extubation.Methods 90 patients with chronic obstructive pulmonary disease admitted to our hospital from December 2018 to January 2019 were selected and divided into study group and control group according to the time of admission,with 45 cases in each group.The control group chose noninvasive mechanical ventilation after extubation,and the study group chose sequential nasal flow oxygen therapy after extubation.The treatment effect,lung function index,oxygenation index,adverse reactions,ICU stay time and death of the two groups were compared.Results The treatment effect of the study group was better than that of the control group(P<0.05).Before treatment,there was no statistically significant difference in FEV1 and FEV1%between the two groups.After treatment,FEV1 and FEV1%in the study group were higher than those in the control group(P<0.05).The oxygenation parameters of the study group were higher than those of the control group(P<0.05).The incidence of adverse reactions in the study group was 11.11%,which was significantly lower than the 33.33%in the control group(P<0.05).The ICU stay time(12.65±0.44)h and mortality(2.22%)in the study group were shorter than those in the control group[(15.89±0.88)h,26.67%](P<0.05).Conclusion Sequential nasal high-flow oxygen therapy after extubation in patients with chronic obstructive pulmonary disease has significant effect,which can significantly improve lung function indexes and oxygenation parameters,prevent reintubation,aspiration,and pressure ulcers,and has high safety.The ICU monitoring stay time is short and the mortality is low,which is worthy of clinical application.
