摘 要:目的了解海淀医院2型糖尿病住院患者用药依从性及自我行为管理现状,评价临床药师日常工作中对糖尿病患者的治疗依从性的效果。方法临床药师在工作中对119名2型糖尿病住院患者进行依从性教育的干预,用中文Morisky用药依从性量表和糖尿病患者自我管理行为量表对患者进行问卷调查,比较干预前后患者依从性和血糖水平的差异。结果干预前和干预后,用药依从性得分分别为(2.51±1.59)和(3.27±1.05)分,自我管理行为得分分别为(35.56±10.85)和(42.59±7.22)分,糖化血红蛋白分别为(9.60±2.58)%和(7.78±1.17)%,空腹血糖分别为(10.54±4.44)和(7.51±1.72)mmol·L^-1,餐后2 h血糖分别为(12.54±5.16)和(8.52±1.41)mmol·L^-1,差异均有统计学意义(均P<0.001)。结论临床药师干预可有效改善2型糖尿病患者的依从性和血糖指标,应对患者进行持续不断地深入随访,以观察更大的临床获益。Objective To measure the current status of medication compliance and self-management behaviors of inpatients with type 2 diabetes in Haidian Hospital.And,to identify the effects of clinical pharmacists’intervention on the compliance of diabetic patients.Methods Clinical pharmacists conducted basic data collection and compliance education for 119 inpatients with type 2 diabetes.Before and after the intervention,Chinese Morisky Medication Compliance Questionnaire and Chinese version Scale of the Diabetes Self-care Activities were used to investigate the patients.The differences on compliance and blood sugar level before and after the intervention were compared.Results Patients’medication compliance scores before and after the intervention were(2.51±1.59)and(3.27±1.05)points,self-management behavior scores were(35.56±10.85)and(42.59±7.22)points,and the hemoglobin A1C were(9.60±2.58)% and(7.78±1.17)%,fasting blood glucose were(10.54±4.44)and(7.51±1.72)mmol·L^-1,2 hours postprandial blood glucose were(12.54±5.16)and(8.52±1.41)mmol·L^-1.The differences were all statistically significant(P<0.001).Conclusion Clinical pharmacists’intervention can effectively improve the medication compliance and blood glucose indexes of patients with type 2 diabetes.Continuous follow-up should be further conducted to collect greater clinical benefits.
