静脉射频腔内闭合系统对大隐静脉曲张并发小腿溃疡的治疗效果

摘　　要：目的:探讨静脉射频腔内闭合系统在大隐静脉曲张并发小腿溃疡疾病中的效果。方法:将2017年1月~2019年8月收治的80例大隐静脉曲张并发小腿溃疡患者的临床资料纳入本次研究,按照入院日期的单双号均分为对照组(大隐静脉高位结扎联合切除剥脱术,40例)与观察组(射频静脉腔内闭合术,40例),对两组患者的治疗效果进行对比与分析。结果:观察组患者的单肢手术时间、溃疡愈合时间以及住院时间均明显短于对照组患者(P值依次为0.001、0.045、0.014),但两组患者的治疗有效率对比无显著差异(x^2=0.042,P=0.838);且在治疗后,观察组患者的疼痛改善程度明显大于对照组患者(t=4.513,P=0.003);在手术的观察与记录中,观察组患者的并发症发生概率明显小于对照组患者(t=4.588,P=0.032);经过1年的随访发现,观察组患者的生活质量评分也显著高于对照组患者(P=0.002);此外,观察组患者在随访过程中的溃疡复发率及曲张复发率与对照组无明显差异(P=0.556)。结论:静脉射频腔内闭合系统在大隐静脉曲张并发小腿溃疡疾病的临床治疗中具有显著的效果,值得临床推广。Objective:To investigate the radiofrequency endovenous closure system offects saphenous varices complicated with leg ulcer disease.Methods:The clinical data of 80 patients with saphenous varicose veins complicated with calf ulcers admitted from January 2017 to August 2019 were included in this study.According to the date of admission,they were randomly divided into control groups(high saphenous vein ligation and stripping,40 cases)and observation group(radiofrequency endovenous closure,40 cases).The treatment of the two groups of patients was compared and analysed.Results:The surgery time,ulcer healing time and hospital stay of the patients in the observation group were significantly shorter than those in the control group(P=0.001,P=0.045,and P=0.014),but there was no significant difference in the treatment efficiency between the two groups(x2=0.042,P=0.838);and after treatment,the degree of pain improvement in the observation group was significantly greater than that in the control group(t=4.513,P=0.003);in the observation and recording of the surgery,the probability of complications in the observation group was significantly less than that in the control group(t=4.588,P=0.032);after one year of follow-up,the quality of life scores of patients in the observation group were significantly higher than those in the control group(P=0.002);in addition,there was no significant difference in the recurrence rates of ulcer and varices between the two groups during the followup(P=0.556).Conclusion:The radiofrequency endovenous closure system has significant application effect in the treatment of saphenous varicose veins complicated with calf ulcer,and it is worthy of clinical promotion.
