摘 要:目的探讨利多卡因抑制舒芬太尼诱发呛咳反射的效果。方法选取2017年1月至2018年10月在我院行腹腔镜胆囊切除患者150例,随机双盲分为利多卡因组(n=74)和生理盐水组(n=76),两组均给予舒芬太尼诱导,其中观察组静脉输注利多卡因,对照组静脉输注等量0.9%氯化钠液,观察两组呛咳发生情况,检测给药前后平均动脉压(MAP)、心律(HR)、血氧饱和度(SpO 2)、气道峰压和肺顺应性。结果观察组呛咳发生率为8.11%,明显低于对照组,差异有统计学意义(P<0.05)。观察组呛咳发生程度明显轻于对照组,差异有统计学意义(P<0.05)。观察组和对照组给药后1 min MAP、HR和SpO 2比较差异无统计学意义(P>0.05)。观察组给药后1 min气道峰压为(19.76±1.84)cmH 2 O,明显低于对照组,而肺顺应性为(37.26±5.11)mL/cmH 2 O,明显高于对照组,差异有统计学意义(P<0.05)。结论利多卡因能有效抑制全麻诱导期舒芬太尼诱发的呛咳反射,值得临床推广。Objective To investigate the effect of lidocaine on inhibiting choking reflex induced by sufentanil.Methods 150 patients who underwent laparoscopic cholecystectomy in our hospital from January 2017 to October 2018 were selected that were randomly and double-blindly divided into lidocaine group(n=74)and normal saline group(n=76).All were given sufentanil induction.Moreover,observation group(OG)was intravenously infused with lidocaine,and control group(CG)was intravenously infused with an equal amount of 0.9%sodium chloride.The occurrence of choking was observed.The mean arterial pressure(MAP),heart rhythm(HR),blood oxygen saturation(SpO 2),peak airway pressure and lung compliance were detected before and after administration.Results The incidence of choking in OG was 8.11%,which was significantly lower than that in CG with statistically significant difference(P<0.05).The incidence of choking in OG was significantly lighter than that in CG with statistically significant difference(P<0.05).There was no significant difference in MAP,HR and SpO 2 between OG and CG at 1 min after administration(P>0.05).The peak airway pressure in OG at 1 min after administration was(19.76±1.84)cmH 2 O,which was significantly lower than that in CG,while the lung compliance was(37.26±5.11)mL/cmH 2 O,which was significantly higher than CG with statistically significant difference(P<0.05).Conclusion Lidocaine could effectively inhibit the cough reflex induced by sufentanil during induction of general anesthesia which is worthy of clinical promotion.
