摘 要:目的探讨甲状腺结节患者甲状腺微波消融术中应用液体隔离技术的效果。方法选取2016年10月至2018年10月收治的100例甲状腺结节患者,根据随机数表法分为两组。所有患者均接受微波消融术治疗,观察组患者则在手术实施期间应用液体隔离技术。对比两组患者术中失血量、手术时间、住院时间、各血清炎症因子[血清C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)及白细胞介素-6(IL-6)]术前、术后水平变化情况及术后并发症发生情况。结果观察组住院时间较对照组短,差异有统计学意义(P<0.05);术后,两组CRP、IL-6、TNF-α水平均较术前均有所升高,但观察组CRP、IL-6、TNF-α各指标水平低于对照组,差异有统计学意义(P<0.05)。观察组术后出血、呼吸困难、喉返神经损伤、甲状旁腺损伤发生率低于对照组,差异有统计学意义(P<0.05)。结论液体隔离技术用于甲状腺微波消融术效果优于单用甲状腺微波消融术,患者手术时间及术中出血量无变化,术后各炎症因子未受到明显影响,且在一定程度上能够缩短其住院时间,降低其术后并发症发生率,有着较高的临床应用价值。Objective To explore the effect of liquidisolationtechnology in thyroidmicrowave ablation.Methods 100 patients with thyroid nodules admitted from October 2016 and October 2018 were selected.According to random number table,these patients were divided into two groups.All patients were treated withmicrowave ablation,while patients in the observation group(OG)were given liquidisolationtechnology during the operation.Intraoperative blood loss,operation duration,hospital stay,serum inflammatory factors[C reactive protein(CRP),tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)]level changes before and after surgery and postoperative complications were compared.Results The hospital stay in OG was shorter than control group(CG)withstatistically significant difference(P<0.05).After operation,CRP,IL-6 and TNF-αwere significantly increased,and these levels in OG were lower than CGwithstatistically significant difference(P<0.05).The incidence of postoperative bleeding,dyspnea,recurrent laryngeal nerve injury and parathyroid gland injury in OG were lower than CG withstatistically significant difference(P<0.05).Conclusion The liquid isolation technology in thyroid microwave ablation is better than single-use thyroid microwave ablation because patient s duration,intraoperative blood loss and postoperative inflammatory factors have not been significantly affected,and their hospital stay and postoperative complications incidence could be reduced,which it has high clinical application value.
