摘 要:目的比较Corvis ST和Pentacam联合诊断系统(CP组合)、A型超声角膜厚度测量仪、非接触式眼压计在测量角膜中央最薄点厚度和眼压的差异性。方法前瞻性研究。选取2019年3~6月在云南省第二人民医院进行角膜屈光手术术前检查的患者作为研究对象,共纳入研究被检者77例(154只眼)。角膜厚度值分别纳入CP组合中Corvis ST角膜顶点厚度值,Pentacam中角膜最薄点厚度值,A型超声角膜测厚仪测量的中央角膜厚度值。眼压分别纳入Corvis ST中直接测量值(IOP)、修正眼压值(BIOP),非接触式眼压计直接测量值(NCT)、联合Pentacam得出的校正眼压值(校正NCT)。厚度数据间差异采用方差分析,眼压数据采用配对t检验,相关性采用Pearson分析,不同检查方法间一致性采用Bland-Altman分析。结果Corvis ST与Pentacam、A型超声超声角膜厚度测量仪所测角膜厚度值组间比较差异有统计学意义(F=3.314,P=0.037);IOP与NCT间、BIOP与校正NCT间比较差异有统计学意义(t=-19.698,P=0.000;t=-15.893,P=0.000)。BIOP与Corvis ST角膜中央厚度无相关性(r=0.088,P=0.259)。Corvis ST与Pentacam、A型超声测量角膜厚度分别有5.56%(8/144)、6.25%(9/144)位于95%一致性界限范围以外;Corvis ST与NCT测量眼压有3.47%(5/144)位于95%一致性界限以外,其中在95%一致性界限内,差值绝对值最大为5.3 mmHg。结论CP组合中Corvis ST对角膜中央厚度值的测量显著高于Pentacam和A型超声测厚仪,Pentacam提供的角膜厚度信息更有意义;Corvis ST对眼压值的测量高于非接触式眼压计,BIOP更能反映眼压真实情况。Objective To compare the difference in the thickness and intraocular pressure(IOP)at the thinnest point in the center of the cornea measured by Corvis ST@Pentacam,A-ultrasound and non-contact tonometer.Methods A total of 77 patients(144 eyes)who underwent preoperative examination before corneal refractive surgery in the second people's hospital of yunnan province from March 2019 to June 2019 were selected in this prospective study.Corneal thickness values included the Corvis ST corneal vertex thickness values,Pentacam corneal thinnest point thickness values,and A-ultrasound central corneal thickness values.IOP values included the Corvis ST directly measured values(IOP),corrected values(BIOP),non-contact tonometer directly measured values(NCT)and combined with Pentacam's corrected values(corrected NCT).Variance analysis was used to assess the differences among the thickness data,paired t test was used for the IOP data,Pearson analysis was used for correlation assessment,and Bland-Altman analysis was used for assessing the consistency among different examination methods.Results The difference among corneal thickness measured by Corvis ST,Pentacam and A-ultrasound corneal thickness measuring instrument was statistically significant(F=3.314,P=0.037).The differences between IOP and NCT as well as between corrected NCT and BIOP were also statistically significant(t=-19.698,P=0.000,t=-15.893,P=0.000).There was no correlation between BIOP and Corvis ST corneal central thickness(r=0.088,P=0.259).5.56%(8/144)of the Corvis ST and Pentacam measurements,and 6.25%(9/144)of the Corvis ST and A ultrasound pachymetry measured corneal thickness was outside the 95%consistency limit,respectively.Moreover,3.47%(5/144)of the IOP measurements by Corvis ST and NCT was outside the 95%consistency limit,with a maximum absolute value of 5.3 mmHg for values within the 95%consistency limit.Conclusions In Corvis ST@Pentacam,the Corvis ST measured central corneal thickness is significantly higher than Pentacam and A-ultrasound pachymetry,and th
