摘　　要：目的观察斜视矫正术后早期患者的散光状态变化。方法对近期本院行斜视矫正手术患者45例(54只眼)进行了前瞻性研究,根据不同手术方式分为3组:Ⅰ组为单眼一条直肌后徙联合一条直肌缩短术35例(35只眼);Ⅱ组为单眼一条直肌后徙联合一条直肌缩短联合下斜肌截断术7例(7只眼);Ⅲ组为单眼一条直肌后徙术9例12只眼)。通过角膜地形图检查对患者术前、术后1 d、1周、2周、4周的散光状态以及角膜中央0,3,5 mm区的鼻、颞侧的角膜曲率进行动态观察分析。结果各组的散光值在手术后早期均有显著差异(P<0.05),1组的角膜曲率在手术后早期均有显著差异(P<0.05),而2组和3组的角膜曲率在手术后早期并无明显差异。结论斜视矫正术在手术后早期可出现患者的散光值的直线上升,不同术式对其影响的程度不同,但在4周后增加散光度数逐渐下降至基本恢复。Objective To evaluate the changes of refractive astigmatism after strabismus surgery at early stage.Method 45 strabismus patients with 54 eyes were perspectively studied.Of those,35 patients received unilateral lateral rectus recession(BLR group,35 eyes)7 patients were received unilateral lateral rectus recession and inferior oblique abduction and 9 patients received unilateral lateral rectus recession and medial rectus resection(R&R group,12 eyes).The refraction status were examined before the surgery and 1 day,1 week,2 weeks and 4weeks after the surgery through corneal topography.Results The astigmatism values of each group were significantly enhanced before and after surgery,corneal curvature in group 1 was significantly different after surgery,while the corneal curvature of group 2 and group 3 was not significantly different after surgery.Conclusion Strabismus surgery may cause transient increased astigmatism,leading to a straight rise while different procedures may lead to different degree and duration.Most can be gradually decreased and recovered.
