直肠黏膜环切联合阴道后壁修补术对直肠前突型便秘的远期疗效分析

摘　　要：目的探讨直肠黏膜环切联合阴道后壁修补术对直肠前突型便秘的远期疗效。方法回顾性分析郧西县中医医院2016年1月—2018年8月收治的82例直肠前突型便秘患者的临床资料,根据手术方法的不同,将82例患者分为两组,每组41例,观察组采用直肠黏膜环切联合阴道后壁修补术,对照组采用直肠黏膜环切术,对比两组患者的围术期指标、临床疗效、术后1个月、6个月及12个月的排便评分。结果观察组的手术时间明显高于对照组(P<0.05),两组患者的术中出血量及24h疼痛评分对比无统计学意义(P>0.05)。观察组治疗有效率100.0%明显高于对照组87.8%(P<0.05)。术后1个月、6个月,观察组排便评分明显低于对照组(P<0.05);术后12个月,两组排便评分对比无统计学意义(P>0.05)。结论直肠黏膜环切联合阴道后壁修补术对可提高直肠前突型便秘的远期疗效,改善排便功能。Objective To explore the long-term curative effect of rectal mucosa circumcision combined with vaginal posterior wall repair on rectocele constipation.Methods the clinical data of 82 patients with rectocele constipation admitted from January 2016 to August 2018 in Yunxi county hospital of traditional Chinese medicine were analyzed retrospectively.According to the different operation methods,82 patients were divided into two groups,41 in each group,the patients in the observation group were treated with rectotomy combined with repair of the posterior vaginal wall,and the patients in the control group were treated with rectotomy.The perioperative parameters,clinical outcomes,and defecation scores at1、6 and 12 months after operation were compared between the two groups.Results The operation time of the observation group was significantly longer than that of the control group(P<0.05).There was no significant difference in the amount of bleeding and 24-hour pain score between the two groups(P>0.05).The effective rate in the observation group was 100.0%higher than that in the control group(87.8%,P<0.05).After 1 month and 6 months,the defecation score in the observation group was significantly lower than that in the control group(P<0.05),and after 12 months,there was no significant difference between the two groups(P>0.05).Conclusion Rectal mucosal circumcision combined with vaginal posterior wall repair can improve the long-term effect of rectocele constipation,and improve defecation function.
