甲状腺手术患者实施针刺麻醉复合丙泊酚芬太尼静脉靶控输注法对血流动力学的影响

摘　　要：目的观察针刺麻醉复合静脉靶控输注丙泊酚芬太尼对甲状腺手术患者血流动力学的影响。方法随机选取莒南县中医医院2014年1月—2019年10月收治的甲状腺手术患者共132例,随机分为两组,每组66例,对照组采用颈丛阻滞麻醉复合氟哌利多芬太尼合剂法麻醉,研究组应用针刺麻醉复合丙泊酚芬太尼静脉靶控输注法麻醉。观察两组不同时间血流动力学变化及麻醉效果。结果研究组患者插管时、手术过程中、拔管时及拔管后5min时的MAP、HR分别高于对照组(P<0.05),研究组麻醉情况优于对照组(P<0.05),研究组steward苏醒评分相对比对照组更高(P<0.05)。结论甲状腺手术采用针刺麻醉复合靶控输注丙泊酚瑞芬太尼麻醉方式有着比较显著的镇痛效果,安全程度也比较高,这一方式值得临床应用。Objective To observe the effect of acupuncture anesthesia combined with propofol fentanyl intravenous target-controlled infusion on hemodynamics in patients undergoing thyroid surgery.Methods A total of 132 patients with thyroid surgery admitted from January 2014 to October 2019 in Junan county hospital of traditional Chinese medicine were randomly divided into two groups,66 patients in each group,the control group was anesthetized by cervical plexus block combined with droperidol fentanyl mixture,and the study group was anesthetized by acupuncture combined with propofol fentanyl intravenous target-controlled infusion.The changes of Hemodynamics and anesthetic effect were observed in two groups at different time.Results Map and HR of the patients in the study group were significantly higher than those in the control group(P<0.05)during intubation,operation,extubation and 5 min after extubation,and the anesthesia of the study group was better than that of the control group(P<0.05),the Steward recovery score in the study group was higher than that in the control group(P<0.05).Conclusion acupuncture anesthesia combined with target-controlled infusion of propofol and remifentanil has a significant analgesic effect and a high degree of safety,which is worthy of clinical application.
