摘  要:目的分析脊柱结核的磁共振成像(magnetic resonance imaging,MRI)表现,探讨MRI诊断脊柱结核的临床价值。方法回顾性分析2011年1月至2019年1月上海交通大学附属第六人民医院收治的经临床和/或手术病理证实的150例脊柱结核患者的MRI表现。结果150例患者中最常见的发病部位是腰椎(72.0%),以相邻双椎体受累(68.7%)最多见。脊柱结核患者MRI表现为椎体及附件骨质破坏、骨髓水肿、信号异常,椎间盘形态改变、信号异常,椎旁脓肿形成,椎管内结构受压等。共147例椎体及附件改变,43例椎间盘改变,114例见椎旁软组织影,62例硬脊膜及脊髓受压。结论MRI能清晰显示脊柱结核的椎体及附件、椎间盘、软组织等各种改变,并能早期明确硬脊膜、脊髓受累范围,对临床早期诊断、鉴别诊断、指导治疗有重要意义。Objective To analyze the MRI findings of spinal tuberculosis and to explore the value of MRI in the diagnosis of spinal tuberculosis.Method A retrospective analysis of MRI findings in 150 patients with spinal tuberculosis confirmed by clinical and/or surgical pathology from January 2011 to January 1919 was performed.Result Of the 150 patients,the most common site of onset is the lumbar spine,and the number of vertebral bodies involved is most common in adjacent vertebral bodies.MRI of spinal tuberculosis patients showed vertebral body and accessory bone destruction,bone marrow edema,signal abnormality,intervertebral disc morphology change,signal abnormality,paraspinal abscess formation,and compression of spinal canal structure.A total of 147 cases of vertebral body and attachment changes,43 cases of intervertebral disc changes,114 cases of paravertebral soft tissue shadow,62 cases of dura mater and spinal cord compression.Conclusion MRI can clearly show the changes of vertebral body and accessory,intervertebral disc and soft tissue of spinal tuberculosis,and can clearly define the extent of spinal cord and spinal cord involvement in early stage.It is of great significance for early clinical diagnosis,differential diagnosis and guidance treatment.
