摘　　要：目的分析上海交通大学医学院附属瑞金医院骨髓移植病区伐昔洛韦的超说明书使用情况,并探讨其安全性管理。方法收集2019年1—12月本院骨髓移植病区所有给予伐昔洛韦治疗的医嘱,共计87份,分析涉及患者的临床诊断、用药方法、用药剂量及用药安全性管理等情况。结果骨髓移植病区伐昔洛韦存在超适应证及超剂量使用情况,超适应证使用患者占100%,超剂量使用患者占73.56%,无不良反应发生。结论伐昔洛韦存在超说明书用药现象,但超说明书用药具有一定的合理性,随着药房信息化自动化管理系统的运用,进一步确保了超说明书用药的安全性。Objective To analyze the use and discuss the safety management of valaciclovir in the bone marrow transplantation area of Ruijin Hospital Affiliated to Shanghai Jiaotong University School of Medicine.Method A total of 87 medical records of valaciclovir from January 2019 to December 2019 used in the bone marrow transplantation area of our hospital were collected.The clinical diagnosis,application method,dosage and safety management of valaciclovir were analyzed.Result Valaciclovir tablets in the bone marrow transplantation area of our hospital had over indication and over dose use,the over indication patients accounted for 100%,the over dose patients accounted for 73.56%,and no adverse reactions were reported.The information-based automatic management was carried out for the drug safety of valaciclovir tablets in our hospital.Conclusion Valaciclovir is off-lable used in the bone marrow transplantation area of our hospital,and it is reasonable to use valaciclovir.The application of pharmacy automation information further ensures the safety of offlable used.
