三酰甘油-葡萄糖指数与高血压病人颈动脉内膜中层厚度的相关性研究

摘　　要：目的探讨三酰甘油-葡萄糖指数(TyG指数)与高血压病人颈动脉内膜中层厚度(cIMT)的关系。方法选取2018年1—12月马鞍山十七冶医院住院的高血压病人157例,检测病人cIMT、空腹血糖、三酰甘油等,计算TyG指数。观察病人cIMT与TyG指数间的关系。结果 157例病人中c IMT增厚者59例(37.6%);cIMT增厚组TyG指数较cIMT正常组高[9.32(8.80,12.61)比8.30(7.21,10.23),P<0.05],病程、体质量指数(BMI)、肥胖比例、吸烟比例、同型半胱氨酸(Hcy)、收缩压、舒张压、空腹血糖、三酰甘油、低密度脂蛋白胆固醇(LDL-C)均较cIMT正常组高(P<0.05);cIMT、Hcy与TyG指数正相关(r=0.199、0.224,P<0.05);logistic回归分析显示TyG指数水平升高增加c IMT增厚的风险,OR=2.675(95%CI:1.518~4.714)。结论动脉粥样硬化是高血压病人主要靶器官损害表现之一,TyG指数水平升高增加高血压病人cIMT增厚风险,可以作为高血压大血管病变评估的简易指标。Objective To explore the association between triglyceride⁃glucose index(TyG)and carotid intima⁃media thickness(cIMT)in patients with hypertension.Methods A total of 157 patients with hypertension in Ma’anshan Shiqiye Hospital from January to December 2018 were selected.The biochemical parameters,including cIMT,fasting blood glucose,triglyceride and so on,were measured and the TyG index was calculated to observe the relationship between patient cIMT and TyG index.Results Among the 157 patients,59(37.6%)suffered from cIMT thickening.The TyG index of the cIMT⁃thickening group was higher than that in the normal group[9.32(8.80,12.61)vs.8.30(7.21,10.23),P<0.05],course of hypertension,body mass index(BMI),the proportion of obesity,the proportion of smoking,homocysteine(Hcy),systolic blood pressure,diastolic blood pressure,FPG,TG,low⁃density lipoprotein cholesterol(LDL⁃C)were higher than those in the normal group(P<0.05);the cIMT and Hcy were inversely correlated with the TyG index(r=0.199,0.224,P<0.05);logistic regression analysis showed that the increase of TyG index in⁃creased the risk of cIMT thickening,OR=2.675(95%CI:1.518⁃4.714).Conclusion Atherosclerosis is one of the main target or⁃gan damage manifestations in patients with hypertension,and the increased level of TyG index increases the risk of cIMT thickening in patients with hypertension,which can be used as a simple index for the assessment of hypertensive macrovascular disease.
