摘 要:目的研究开放性骨折患者创伤部位发生感染的病原菌特点及对抗生素的敏感性。方法采集124例开放性骨折创伤部位发生感染的患者的创面脓性分泌物,进行病原菌培养鉴定和药敏分析。结果124例开放性骨折患者的创面感染脓性分泌物共培养出132株病原菌,其中革兰阳性菌46株,占34.85%,革兰阴性菌86株,占65.15%。革兰阴性球菌的铜绿假单胞菌、大肠埃希菌和鲍氏不动杆菌主要对美罗培南敏感;革兰阳性菌的金黄色葡萄球菌、表皮葡萄球菌和粪肠球菌对青霉素和红霉素的耐药率高,对万古霉素和亚胺培南敏感。结论开放性骨折创面感染的病原菌以革兰阴性菌的铜绿假单胞菌、大肠埃希菌和鲍氏不动杆菌为主,临床可以参考药敏结果选择合适的抗生素。Objective To study the characteristics of pathogens in the wounds of patients with open fractures and their sensitivity to antibiotics.Methods The purulent exudates of 124 patients with trauma site infection of open fractures were collected for pathogen culture identification and drug sensitivity analysis.Results A total of 132 pathogenic bacteria were cultured in the wounds of 124 patients with open fractures,including 46 strains of Gram-positive bacteria,accounting for 34.85%,and 86 strains of Gram-negative bacteria,accounting for 65.15%.Gram-negative cocci,Pseudomonas aeruginosa,Escherichia coli and Acinetobacter baumannii are mainly sensitive to meropenem,and the gram-positive bacteria Staphylococcus aureus,Staphylococcus epidermidis and Enterococcus faecalis showed high drug resistance to penicillin and erythromycin,and were sensitive to vancomycin and imipenem.Conclusion The pathogens of open fracture wound infection are mainly Gram-negative bacteria,such as Pseudomonas aeruginosa,Escherichia coli and Acinetobacter baumannii.Clinically,the appropriate antibiotics can be selected with reference to drug sensitivity results.
