封闭式载杆与开放式载杆冷冻胚胎的效果比较

摘　　要：目的:探讨封闭式载杆与开放式载杆冷冻胚胎的效果。方法:回顾性分析2016年1月至2017年12月在我院行体外受精—胚胎移植(IVF-ET)技术助孕445例行全胚冷冻患者(年龄<37岁)首次胚胎解冻移植的临床资料,按冷冻载体不同分为两组。A组(n=171)行常规促排卵,IVF受精,第3 d使用封闭式载杆冷冻1枚以上Ⅰ级胚胎和Ⅱ级胚胎;B组(n=274)行常规促排卵,IVF受精,第3 d使用开放式载杆冷冻1枚以上Ⅰ级胚胎和Ⅱ级胚胎。对比两组胚胎复苏情况、移植情况、子代出生情况。结果:开放式载杆组的活产率显著高于封闭式载杆(P<0.05)。两组的胚胎损伤率、胚胎种植率、流产率、活产率、早产率、出生缺陷率和出生男女比率差异无统计学意义(P>0.05)。结论:开放式载杆冷冻胚胎、封闭式载杆冷冻胚胎均安全有效,各有优缺点。前者保存胚胎较后者有更高的活产率,后者保护胚胎免受液氮中微生物的污染和交叉感染。临床可根据患者情况和各中心情况选择适宜的冷冻载杆。Objective:To investigate the effect of closed and open frozen carriers on the vitrification of embryos.Methods:Clinical data of 445 patients undergoing in vitro fertilization-embryo transfer(IVF-ET)in Ganzhou people’s hospital between Jan.2016 and Dec.2017 were retrospectively analyzed.These patients were divided into two groups based on the frozen carriers used,including group A with 171 cases and group B with 274 cases.Ovulation induction and IVF insemination were routinely conducted for patients in both groups,and on Day 3,>1 embryos of grade I and gradeⅡwere frozen with closed frozen carriers in group A and with open frozen carriers in group B.Embryo recovery rates,transplantation outcomes and live both rates were compared between the two groups.Results:Compared to closed carriers,the open frozen carriers group had higher clinical pregnancy rate.The clinical pregnancy rate were significantly higher in the open frozen carriers group(P<0.05).The recovery rate,embryo-ininjured rate,implantation rate,miscarriage,birth defect rate,premature birth rate,male to female ratio were similar in two groups(P>0.05).Conclusion:There are advantages and disadvantages for open and closed frozen carriers in embiyo cryopreservation,and the live birth rate is slightly higher for the open frozen carriers.The closed carriers can protect the embroy from microbial contamination and cross-infection in liquid nitrogen.Cryopreservation carriers can be selected according to the patient s condition and clinical characteristics in each IVF center.
