超声造影评估2型糖尿病肾病和膜性肾病血流灌注的临床价值

摘　　要：目的利用超声造影对2型糖尿病肾病和膜性肾病的血流灌注进行定量评估,为临床提供辅助诊断价值。资料与方法前瞻性纳入31例拟行肾脏穿刺的肾病患者,进行超声造影检查,记录肾实质达峰时间(TP)、峰值强度(PEAK)、平均渡越时间(mTT)、肾血流量曲线下面积,并与病理结果进行对比。结果31例患者中,病理诊断糖尿病肾病19例,膜性肾病12例;糖尿病肾病组造影的PEAK低于膜性肾病组[(24.37±7.16)%比(29.77±6.42)%,P<0.05];两组TP、mTT、肾血流量曲线下面积比较,差异无统计学意义(P>0.05)。结论超声造影能实时动态评估肾脏的血流灌注,造影参数PEAK对于评估糖尿病肾病及膜性肾病具有一定的临床意义。Purpose To use contrast-enhanced ultrasound to quantitatively evaluate the blood perfusion of type 2 diabetic nephropathy and membranous nephropathy,and to provide clinical auxiliary diagnostic value.Materials and Methods Thirty-one patients with nephropathy who underwent renal puncture were prospectively enrolled in this study,all patients underwent contrast-enhanced ultrasound examination,renal parenchymal time to peak(TP),peak intensity(PEAK),mean transit time(mTT)and area under the renal blood flow curve were recorded,and compare with the pathological results.Results Among the 31 patients,19 were pathologically diagnosed as diabetic nephropathy and 12 with membranous nephropathy.The PEAK of the diabetic nephropathy group was lower than that of the membranous nephropathy group[(24.37±7.16)%vs.(29.77±6.42)%,P<0.05];there was no significant difference in TP,mTT and area under the renal blood flow curve between the two groups(P>0.05).Conclusion Contrast-enhanced ultrasound can dynamically evaluate the blood perfusion of the kidney in real time.The contrast-enhanced parameter PEAK has certain clinical significance for the evaluation of diabetic nephropathy and membranous nephropathy.
