中医鼻病序贯疗法对儿童中耳炎疗效观察

摘　　要：目的探讨中医鼻病序贯疗法治疗儿童分泌性中耳炎(SOM)的临床价值。方法 82例SOM患儿,随机分为序贯组(41例,56耳)与对照组(41例,54耳)。对照组采用阿奇霉素、桉柠蒎、丙酸氟替卡松治疗,疗程4周;序贯组采用中医鼻病序贯疗法,疗程4周。比较2组治疗前后的中医证候总积分、鼓室压曲线、总有效率、不良反应与复发率。结果经治疗后,2组耳胀、耳痛积分、中医证候总积分均降低(P<0.05),序贯组低于对照组(P<0.05)。治疗后,序贯组A型曲线的占比明显高于对照组(P<0.05)。序贯组总有效率(82.14%,46/56)高于对照组(62.96%,34/54)(P<0.05)。2组治疗期间均未见严重不良反应。序贯组随访期间复发率(2.44%,1/41)低于对照组(19.51%,8/41)(P<0.05)。结论中医鼻病序贯疗法治疗儿童SOM的疗效优于单纯西药治疗,且复发率相对更低。Objective To explore the clinical value of TCM sequential therapy of nasal diseases in children with secretory otitis media (SOM).Methods A total of 82 children of SOM were divided into sequential group (41 patients,56 ears) and control group (41 patients,54 ears).The control group was treated with azithromycin,eucalyptus citrus,fluticasone propionate for 4 weeks.The sequential group used TCM sequential therapy of nasal diseases for 4 weeks.TCM total score,tympanic pressure curve,total effective rate,adverse reaction and recurrence rate were compared between two groups.Results The ear swelling,ear pain scores,and TCM syndrome scores of the two groups were lower than those before treatment (P<0.05),the sequential group was lower than control group (P<0.05).After treatment,the proportion of the A-type curve in sequential group was significantly higher than that in the control group (P<0.05).The total effective rate of the sequential group was 82.14% (46/56),which significantly higher than 62.96% (34/54) in control group (P<0.05).No serious adverse reactions were observed during the two treatment periods.The recurrence rate during the follow-up of the sequential group was 2.44% (1/41),which lower than 19.51% (8/41) in the control group (P<0.05).Conclusion The curative effect of TCM sequential therapy of nasal diseases for children with SOM is better than that of Western medicine alone,and the recurrence rate is relatively lower.
