盐酸氨基葡萄糖关节腔注射联合仙灵骨葆胶囊治疗关节炎的临床观察

摘　　要：目的:观察盐酸氨基葡萄糖关节腔注射联合仙灵骨葆胶囊治疗关节炎的临床疗效,及对患者血清5-羟色胺(5-HT)、软骨基质金属蛋白酶-3(MMP-3)、基质金属蛋白酶抑制剂-1(TIMP-1)水平的影响。方法:124例关节炎患者随机分为对照组和观察组各62例。对照组给予口服仙灵骨葆胶囊治疗,观察组在对照组基础上加用盐酸氨基葡萄糖关节腔注射。治疗4周后比较两组患者临床疗效和药品不良反应发生情况,以及两组患者治疗前后血清5-HT、MMP-3、TIMP-1水平变化。结果:观察组临床有效率为93.54%,明显高于对照组的79.03%(P<0.05)。治疗后两组患者的VAS评分和肿胀评分,以及5-HT、MMP-3水平均较治疗前明显下降,TIMP-1水平则较前明显升高(P<0.05);且观察组各项指标均优于对照组(P<0.05)。观察组治疗后的日常生活能力明显优于对照组(P<0.05)。两组药品不良反应发生率差异无统计学意义(P>0.05)。结论:盐酸氨基葡萄糖关节腔注射联合仙灵骨葆胶囊治疗关节炎疗效显著,可明显改善患者临床症状,提高日常生活能力,且安全性较高。Objective: To study the effect of glucosamine hydrochloride for joint cavity injection combined with Xianlinggubao capsules on the levels of 5-HT,MMP-3 and TIMP-1. Methods: Totally 124 patients with arthritis were randomly divided into the control group and the combined group with 62 cases in each group. The control group was treated with Xianlinggubao capsules,while the combined group was treated with glucosamine hydrochloride with joint cavity injection and Xianlinggubao capsules. The clinical efficacy and adverse reactions were compared between the two groups. The serum levels of 5-HT,MMP-3 and TIMP-1 were compared before and after the treatment.Results: The clinical effective rate of the control group and the combined group was 79.03% and 93.54%,respectively with statistical significance( P<0.05). There were no significant differences in VAS score and swelling score between the two groups before the treatment( P>0.05). After the treatment,the scores all decreased,and those in the combined group were significantly lower than those in the control group with statistical significance( P<0.05). After the treatment,the daily living ability of the combined group was significantly better than that of the control group( P<0.05). There were no significant differences in the serum 5-HT,MMP-3 and TIMP-1 levels between the two groups before the treatment( P>0.05). After the treatment,the levels of 5-HT and MMP-3 decreased,and the level of TIMP-1 in the combined group was significantly higher than that in the control group( P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups( P> 0.05). Conclusion: The treatment of arthritic with glucosamine hydrochloride combined with Xianlinggubao capsules has a significant effect,which can significantly improve the clinical symptoms and improve the ability of daily life with high safety.
