甘麦大枣汤改善肿瘤抑郁的临床疗效

摘　　要：目的:观察甘麦大枣汤治疗肿瘤抑郁的临床疗效。方法:选取2018年1月至2018年10月上海中医药大学附属龙华医院收治的肿瘤抑郁患者72例作为研究对象,按照随机数字表法分为对照组和观察组,每组36例。2组均给予抗肿瘤中成药消癌平片,观察组加用甘麦大枣汤。2组疗程均为28 d,比较2组治疗前后抑郁自评量表(SDS)评分,抑郁相关指标5-羟色胺(5-HT),同型半胱氨酸(Hcy),肿瘤坏死因子-α(TNF-α),白细胞介素-6(IL-6)、自然杀伤细胞(NK)、免疫学指标(CD3^+、CD4^+、CD8^+、CD4^+/CD8^+)水平及生命质量评分(QLQ-C30)的变化情况。结果:治疗后,SDS评分观察组明显低于对照组,差异有统计学意义(P<0.05)。抑郁相关炎性反应递质指标5-HT、TNF-α、IL-6改善,观察组好转程度明显高于对照组,差异有统计学意义(P<0.05)。免疫学指标CD3^+、CD4^+、CD4^+/CD8^+水平观察组改善程度明显高于对照组,差异有统计学意义(P<0.05)。生命质量量表中躯体、角色、情绪、社会功能、总健康状况、疲倦、失眠项目评分差异有统计学意义(P<0.05)。结论:甘麦大枣汤联合消癌平片治疗肿瘤抑郁患者,可明显减轻抑郁,提高机体免疫功能,改善生命质量。Objective:To observe the clinical efficacy of Ganmai Dazao Decoction in treating cancer-related depression.Methods:A total of 72 patients with cancer-related depression admitted to Longhua Hospital Affiliated to Shanghai University of Traditional Chinese Medicine were randomly divided into a control group and an observation group,with 36 cases in each group.Both groups were treated with Xiaoaiping Tablets,a kind of Chinese patent medicine.And the observation group was added with Ganmai Dazao Decoction.The course of the treatment for the 2 grous were 28 days.The Self-rating depression scale(SDS)score,the levels of 5-HT,Hcy,TNF-α,IL-6,the levels of natural killer(NK)cells and T cells(CD3^+,CD4^+,CD8^+,CD4^+/CD8^+)levels and the changes of QLQ-C30(V3.0)were compared.Results:The SDS score of the observation group was obviously lower than that of the control group and the difference was statistically significant(P<0.05).After treatment,there were statistically significant differences on the improvement levels of 5-HT,TNF-α,IL-6 between the 2 groups,and the improvement in the observation group was obviously better than that in the control group(P<0.05).After treatment,there were statistically significant differences on the immunity index levels of CD3^+,CD4^+and CD4^+/CD8^+between the 2 groups.The score difference in physical function,role function,emotional function,social function,total health status,fatigue,and insomnia score of life quality was statistically significant between 2 groups after treatment(P<0.05).Conclusion:Ganmai Dazao Decoction combined with Xiaoaiping Tablets can be used in the treatment of cancer-related depression,which can significantly alleviate the depression,and improve immune function and quality of life.
