摘  要:目的探究CD14+HLA-DR-/low髓系抑制性细胞(myeloid-derived suppressor cell,MDSC)表达水平在COPD患者疾病进展中的诊断价值。方法选取本院2018年1月至2019年10月收治的呼吸系统疾病患者56例,依据临床诊断分为COPD疾病组(n=40)和对照组(n=16),COPD组分为急性加重期组(n=20)和稳定期组(n=20),采集患者不同疾病时期和疾病治疗前后的外周血检测MDSC、CRP、ESR、WBC水平,分析与COPD患者疾病进展中的诊断价值。结果急性加重组MDSC、CRP、ESR、WBC水平与对照组比较差异具有统计学意义(P<0.05);稳定期组MDSC、CRP、ESR水平与对照组比较差异具有统计学意义(P<0.05);稳定期组与加重期组MDSC、CRP、ESR、WBC水平比较差异具有统计学意义(P<0.05);治疗后急性加重组患者,MDSC表达水平下降,MDSC阳性细胞数明显减少,治疗前后比较差异具有统计学意义(P<0.05)。结论 MDSC单核系外周血表达水平可作为判断COPD疾病进展的有效指标,联合CRP、ESR、WBC检测可为临床治疗和判断病情提供依据。Objective To explore the diagnostic value of CD14+HLA-DR-/lowmyelin inhibitory cell(MDSC) expression level in COPD patients with disease progression. Methods 56 patients with respiratory system diseases in our hospital from January 2018 to October 2019 were seleted,they were divided into COPD disease groups(n=40) and the control group(n=16) on the basis of clinical diagnosis, and COPD disease groups divided into acute aggravating group(n=20) and stability group(n=20). The levels of MDSC, CRP, ESR and WBC in peripheral blood were detected before and after treatment, and the diagnostic value of MDSC, CRP, ESR and WBC in COPD patients was analyzed. Results The levels of MDSC,CRP, ESR and WBC in the acute aggravating group were significantly difference compared with those of the control group(P<0.05). The MDSC,CRP and ESR levels in the stability group were significant difference(P<0.05). The MDSC, CRP, ESR and WBC levels of the stability group and the aggravating group was significant difference(P<0.05). After treatment, the expression level of MDSC decreased, and the number of MDSC positive cells significantly decreased, the difference before and after treatment was statistically significant(P<0.05). Conclusion MDSC mononuclear peripheral blood expression level can be used as an effective indicator to determine the progression of COPD disease, combined with CRP, ESR,WBC detection can provide a basis for clinical treatment and disease judgment.
