摘　　要：目的探讨急性阑尾炎行腹腔镜阑尾切除术(laparoscopic appendectomy,LA)的效果。方法回顾性分析本院2013年10月至2017年6月收治的243例行腹腔镜阑尾切除术的急性阑尾炎患者的临床资料,戳孔选择三孔法,分析手术结果。结果242例患者手术获成功,其中中转开腹1例,切口感染4例,腹腔残余感染12例;手术时间20~130 min,平均(41.35±19.78)min;术中放置右下腹胶管引流68例;术后使用抗菌药物时间2~11 d,平均(3.54±1.63)d;术后肛门排气时间6~30 h,平均(10.21±3.79)h;住院时间3~12 d,平均(4.19±1.16)d;无腹腔出血、肠漏、肠梗阻、阑尾残株炎、切口疝等并发症发生。结论腹腔镜阑尾切除术可全面探查腹腔,具有创伤小、恢复快、住院时间短、并发症少、疼痛轻、美容效果好等优点,值得临床推广运用。Objective To investigate the effect of laparoscopic appendectomy(LA)for acute appendicitis.Methods The clinical data of 243 patients with acute appendicitis who underwent laparoscopic appendectomy from October 2013 to June 2017 in our hospital were retrospectively analyzed.The three hole method were selected for the puncture.The results were analyzed.Results The operation was successful in 242 cases patients,including 1 case transferred to laparotomy,4 cases infected by incision and 12 cases infected by residual abdominal infection.The operation time was 20-130 min,with an average of(41.35±19.78)min.Intraoperative right lower abdomen catheter drainage was performed in 68 cases.The duration of postoperative antibiotic use was 2-11 d,with an average of(3.54±1.63)d.Postoperative anal exhaust time was 6-30 h,with an average of(10.21±3.79)h.The length of hospital stay was 3-12 d,with an average of(4.19±1.16)d.No complications such as abdominal hemorrhage,intestinal leakage,intestinal obstruction,residual appendicitis,incisional hernia occurred.Conclusion Laparoscopic appendectomy can fully explore the abdominal cavity,and has the advantages of less trauma,faster recovery,shorter hospital stay,fewer complications,less pain,and better cosmetic effect,which is worthy of clinical application.
