自体脂肪移植结合化妆光影原理改善面部轮廓的临床研究

摘　　要：目的介绍面部自体脂肪移植结合化妆光影原理改善面部轮廓的临床效果。方法自2015年1月至2018年12月,对面部自体脂肪移植术的43例女性患者,依据面部化妆光影原理中的"视错觉"效应进行患者脸形的术前评估,标记出面部高光区和修容区。采取Coleman脂肪处理并依据面部各脂肪室分区对其高光区进行各分区的自体脂肪填充,收集术后随访6个月以上患者的照片并评价手术效果。结果43例患者术后获随访6~24个月,其中单次注射32例,二次注射8例,三次注射3例。所有患者术后均恢复良好,术后无脂肪液化、感染、脂肪栓塞等并发症发生,面部饱满程度上得到改善的同时在面部年轻化效果上也有一定改善,患者总满意率达93.02%。结论将化妆光影原理应用于面部自体脂肪移植术前美学设计中可以依据不同脸形更加准确地判断填充区域及范围,使手术效果立体精致、过渡自然。Objective To introduce the clinical efficacy of autologous fat transplantation combined with light and shadow principle of cosmetic in the improvement of facial contour.Methods From January 2015 to December 2018,43 female patients underwent autologous fat transplantation of face.According to the"visual illusion"effect of light and shadow principle of cosmetic,the facial contour was evaluated and the facial highlight areas and modified areas were marked before the operation.The fat was treated based on Coleman’s method.According to the areas of facial fat compartments,autologous fat transplantation was performed in each division of highlight area.Postoperative follow-up photos over 6 months were collected the efficacy of autologous fat transplantation was evaluated.Results Forty-three patients were followed up from6 to 24 months.Thirty-two patients received single transplantation,8 patients received secondary transplantation and 3 patients received the third transplantation.All patients recovered well after operation.No complications such as fat liquefaction,infection and fat embolism occurred.The efficacy was improved not only in facial fullness but also in facial rejuvenation.The total satisfaction rate of patients was 93.02%.Conclusion The application of light and shadow principle of cosmetic in the preoperative aesthetic design of facial autologous fat transplantation could accurately judge the filling area and scope according to different facial contours.The postoperative appearance was natural.
