后路钉棒系统固定融合术治疗类风湿性寰枢椎脱位的疗效分析

摘　　要：目的:分析类风湿性寰枢椎脱位通过后路钉棒系统固定融合术治疗的效果和应用。方法:选取本院于2015年1月—2019年1月脊柱外科接收的类风湿性寰枢椎脱位患者22例,对患者的术前及术后6个月VAS评分、JOA评分情况进行分析。结果:通过对22例患者术前及术后6个月JOA评分、VAS评分进行分对发现,患者在术前VAS评分为(5.72±1.54)分,术后6个月VAS评分为(2.28±0.57)分,术后6个月的VAS评分显著低于术前(P<0.05),患者在术前JOA评分为(12.37±2.26)分,术后6个月JOA评分为(15.69±2.34)分,术后6个月的JOA评分显著高于术前(P<0.05)。术后对所有患者进行X线和CT影像学检查,22例患者均获解剖复位,寰枢椎均获得可靠融合,内固定无松动。结论:后路钉棒系统固定融合术治疗类风湿性寰枢椎脱位的效果较为理想,可获得满意的临床疗效。Objective To study the effect and application of posterior screw rod system in the treatment of rheumatoid atlantoaxial dislocation.Methods from January 2015 to January 2019,22 patients with rheumatoid atlantoaxial dislocation received by spine surgery in our hospital were selected.The relevant data of all patients were recorded and analyzed retrospectively.The VAS score and JOA score of 22 patients before and 6 months after operation were studied and analyzed.Results by comparing the scores of JOA and vas of 22 patients before and 6 months after operation,we found that the score of VAS was(5.72±1.54)before operation and(2.28±0.57)after operation.The score of vas of 6 months after operation was significantly lower than that before operation(P<0.05).The score of JOA before operation was(12.37±2.26).The score of JOA after operation was(15.69±2.34).The score of JOA after operation for 6 mothons was significantly higher than that before operation(P<0.05).All patients were examined by X-ray and CT imaging.22 patients were anatomically reduced,atlantoaxial fusion was reliable,and internal fixation was not loose.Conclusion the treatment effect of posterior screw rod system fixation and fusion in this study is ideal,and it can obtain satisfactory clinical effect.
