摘  要:目的探讨Notch信号转导通路在与老年患者膝关节骨性关节炎的相关性。方法选取老年膝骨关节炎并屈曲内翻畸形采取人工全膝关节置换术的患者35例,作为观察组;选取膝关节外伤行关节镜手术治疗患者13例,作为对照组。观察组取术中截骨成形去除的股骨髁负重面的退变软骨组织,对照组取因外伤脱落需要去除的股骨髁软骨组织。进行HE染色、番红O-固绿染色,观察软骨组织病变情况及免疫组织化学检测Notch1、Jagged1的表达情况。结果从组织病理学观察,观察组软骨表面粗糙变薄,软骨细胞数目大量减少,细胞排列不整齐,潮线不连续,软骨基质染色明显不均匀;对照组软骨面光滑,细胞排列整齐,潮线完整连续,软骨基质染色均匀。番红O-固绿染色结果,观察组软骨细胞数明显减少,排列成簇,番红染色深染或失染,钙化层变薄,潮线间隙增宽;对照组软骨细胞排列较均匀,番红染色轻染,潮线连续。免疫组化结果显示,观察组中Notch1为0.51±0.14、Jagged1为0.45±0.07;观察组中Notch1为0.22±0.11、Jagged1为0.27±0.09,观察组与对照组比较差异有统计学意义。结论Notch信号转导通路与老年骨关节炎膝关节软骨退行性改变密切相关,可能参与调控老年膝骨关节炎发生、发展的病理改变。Objective To investigate the expression of Notch signal transduction pathway in osteoarthritis cartilage of the elderly patients and the correlation between Notch signal transduction pathway and osteoarthritis.Methods From October 2018 to January 2020,35 elderly patients were selected as the observation group,who were treated with total knee arthroplasty due to knee osteoarthritis and flexion varus deformity,and 13 patients were selected as the control group who were treated with arthroscopic surgery due to knee trauma. In the observation group,the cartilage of the load-bearing surface of the femoral condyle was removed by osteotomy,while in the control group,the cartilage of the femoral condyle was removed due to trauma. HE staining and Safranine O-fixation green staining were used to observe the pathological changes of cartilage and the expression of Notch 1 and Jagged1 were observed by immunohistochemistry.Results According to the histopathological observation,the surface of cartilage in the observation group was rough and thin,the number of chondrocytes was greatly reduced,the arrangement of cells was irregular,the tide line was discontinuous,and the staining of cartilage matrix was obviously uneven. The surface of cartilage in the control group was smooth,the arrangement of cells was orderly,the tide line was complete and continuous,and the staining of soft bone matrix was even. The results of safranin o-fixation green staining showed that the number of chondrocytes in the observation group was significantly reduced and arranged in clusters,the calcified layer became thinner,and the intertidal space widened,while the arrangement of chondrocytes in the control group were evenly arranged,lightly stained with crocin,and the intertidal line was continuous. The results of immunohistochemistry showed that Notch1 0.51±0.14,Jagged1 0.45±0.07 in the observation group and Notch1 0.22±0.11,Jagged1 0.27±0.09 in the control group.
