摘 要:目的探索25羟维生素D[25(OH)D]对体外受精(IVF)的影响。方法68例接受体外受精-胚胎移植的患者,均采用卵泡期高孕激素状态下促排卵方案(PPOS)。按25(OH)D正常与否分为研究组和对照组2组,比较2组间年龄、体重指数(BMI)、基础促卵泡激素(FSH)、促黄体生成素(LH)、雌二醇(E2)、睾酮(T)、抗苗勒管激素(AMH)、Gn天数、Gn量、取卵数、MII卵数、受精数、优胚数以及最大卵泡达14 mm前、14 mm到HCG日卵泡大小平均日增长的差异性。结果25(OH)D与基础E 2和14 mm前卵泡日增长呈正相关性(P<0.05),与年龄、BMI、基础FSH、LH、T、AMH、Gn天数、Gn量、取卵数、MII卵数、受精数、优胚数以及14 mm到HCG日卵泡日增长之间无显著相关性(P>0.05);对照组的E 2和14 mm前卵泡日增长显著高于研究组,差异有统计学意义(P<0.05);2组间年龄、BMI、基础FSH、LH、T、AMH、Gn天数、Gn量、取卵数、MII卵数、受精数、优胚数以及14 mm到HCG日卵泡日增长差异无统计学意义(P>0.05)。结论25(OH)D与E 2代谢和卵泡早期募集与优势比有关,但不影响IVF过程中的获卵数、MII卵成熟数、受精率和优胚率。Objective To explore the effect of 25 hydroxyvitamin D 25(OH)D on in vitro fertilization.Methods 68 cases of IVF-ET(in vitro fertilization-embryo transfer)patients were selected and divided into the study group and the control group according to whether 25(OH)D was normal or not.Age,Body mass index(BMI),Basal follicle stimulating hormone(FSH),Luteinizing hormone(LH),Estradiol(E 2),Testosterone(T),anti-Mullerian hormone(AMH),Gn days were compared between the two groups.The difference of the average daily growth of follicle size before the maximum follicle reached 14 mm,14 mm to HCG was observed.Results There was a positive correlation between 25(OH)D and the daily growth of basal E2 and follicle before 14 mm(P<0.05),and it was no significant correlation with age,BMI,basal FSH,LH,T,AMH,Gn days,the number of oocytes collected,the number of MII oocytes,the number of fertilized embryos,the number of superior embryos and the daily growth of follicles from 14mm to HCG(P>0.05).The daily growth of follicles before 14 mm and E 2 in the control group were significantly higher than those in the study group(P<0.05).The age,BMI,basic FSH,LH,T,AMH,Gn days,the number of oocytes collected,the number of MII oocytes,the number of fertilized embryos,the number of superior embryos and the daily growth of follicles from 14 mm to HCG have no significant difference between the two groups(P>0.05).Conclusion 25(OH)D is related to E 2 metabolism and early follicular recruitment and dominance,but it does not affect the number of oocytes obtained,the number of MII oocytes matured,the fertilization rate and the rate of excellent embryo during IVF.
