针刺加红外线照射联合骨痛膏治疗高龄患者腰腿痛临床研究

摘　　要：目的:探讨针刺加红外线照射联合骨痛膏治疗高龄患者腰腿痛的临床疗效及对炎症介质的影响。方法:选取154例高龄腰腿痛患者作为研究对象,通过随机数字表法分为对照组和观察组各77例。对照组给予单纯止痛处理,观察组则给予针刺加红外线照射联合自制骨痛膏贴敷,两组疗程均为10 d。观察比较两组患者治疗前后的数字分级评分法(NRS)得分、日本骨科协会(JOA)评分、Oswestry功能障碍指数(ODI)、血清P物质、血清白细胞介素-6(IL-6)变化,判断针刺加红外线照射联合骨痛膏对高龄患者腰腿痛的临床疗效。结果:观察组患者的临床总有效率(90.9%)明显高于对照组(77.9%),差异具有统计学意义(P<0.05)。两组患者NRS得分、ODI较治疗前均有明显下降(P<0.01),JOA评分明显升高(P<0.01),但观察组较对照组改善更为显著,差异具有统计学意义(P<0.01)。两组患者P物质、IL-6均较治疗前改善(P<0.01),观察组表现优于对照组,差异具有统计学意义(P<0.01)。结论:针刺加红外线照射联合骨痛膏贴敷治疗高龄患者腰腿痛,可显著改善其临床症状,提高生活质量,其机制可能与减轻局部炎症反应有关。Objective:To explore the effect of acupuncture plus infrared irradiation combined with Gutong Plaster(骨痛膏) on lumbocrural pain and inflammatory mediators in elderly patients. Methods:There were 154 elderly patients with lumbocrural pain included as the study object. Using a random table,patients were divided into observation group and control group,77 patients in each group. The control group was only given treatment to relieve pain and the observation gro up ture plus infrared irradiation combined with Gutong Plaster. Both groups were treated for 10 days. The clinical efficacy was evaluated through observing changes of numeric rating scales(NRS) score,Japanese Orthopedic Association(JOA) score,Oswestry dysfunction index(ODI) and the content of serum substance P(SP) and interleukin 6(IL-6) in two groups. Results:After the treatment,clinical efficiency in the observation group(90.9%) was higher than that in the control group(77.9%)(P<0.05). After the treatment,NRS score and ODI in two groups were reduced obviously(P<0.01),and JOA score improved obviously(P<0.01),and the changes were more obvious in the observation group(P<0.01). After the treatment,contents of serum SP and IL-6 in two groups were improved(P<0.01),and in the observation the increase was more than that in the control group(P<0.01). Conclusions:Acupuncture plus infrared irradiation combined with Gutong Plaster on lumbocrural pain can significantly improve the clinical symptoms and the quality of life in elderly patients. The possible mechanism might be that this treatment could reduce local inflammation.
