摘 要:目的分析轻度认知障碍(mild cognitive impairment,MCI)患者的生存率以及MCI转化为阿尔茨海默病(alzheimer's disease,AD)的影响因素。方法对113名MCI患者的影响因素采用非参数检验进行单因素分析,进一步采用Cox回归进行多因素分析。结果在2005-2014年的10年内MCI患者在3、6、24、36、48、96、120个月生存率依次为99.1%、98.2%、97.3%、87.1%、64.1%、33.4%、0.9%,其中位生存时间为72个月;单因素分析得到MCI患者的生存率在年龄、性别、APOEε4、FAQ、CDRSB、MMSE中有统计学差异(P<0.05);多因素分析显示高龄(HR=1.007, 95%CI:1.020~1.138)、女性(HR=1.849, 95%CI:1.101~3.105)、高CDRSB得分(HR=1.171, 95%CI:1.045,1.312)为MCI转化为AD的危险因素。结论 MCI患者的生存率较低,且年龄、性别和CDRSB得分对MCI进展为AD具有重要意义。Objective The aim of this study was to evaluate the survival rate of patients with mild cognitive impairment(MCI)and to find the factors of mild cognitive impairment progression to Alzheimer’s disease(AD).Methods Nonparametric tests were used for univariate analysis and COX regression was performed for survival analysis for the factors of 113 MCI patients.Results The survival rate of MCI patients were 99.1%,98.2%,97.3%,87.1%,64.1%,33.4%,and 0.9%at 3,6,24,36,48,96,and 120 months,respectively.And the median survival time was 72 months.The univariate analysis showed that there were significant differences in age,gender,APOEε4,FAQ,CDRSB,and MMSE in the aspects of the survival rate of MCI patients(P<0.05).Multivariate analysis showed that the potential predictors included elder(HR=1.007,95%CI:(1.020,1.138)),female(HR=1.849,95%CI:(1.101,3.105)),and high CDRSB scores(HR=1.171,95%CI:(1.045,1.312)).Conclusion The rate of MCI patients was low.Age,gender,and the CDRSB scores were important factors of progression to AD in MCI patients.
