中越边境地区嫖客人群HIV/STIs感染相关因素分析

摘　　要：目的了解中越边境地区嫖客的HIV/STIs感染现状,探索HIV/STIs感染相关因素。方法 2015-2016年,在云南省河口县开展横断面调查,收集嫖客的人口学特征、性行为特征和毒品使用行为等信息;实验室检测嫖客HIV/STIs感染和毒品使用;多因素logistic回归模型筛选HIV/STIs感染的相关因素。结果嫖客年龄中位数36周岁,HIV感染率为2.06%,梅毒和HSV-2感染率分别为0.64%和16.49%。多因素分析分析发现,年龄50周岁及以上(OR=8.11, 95%CI:1.26~52.16)和吗啡阳性(OR=7.35, 95%CI:1.42~38.06)是嫖客人群HIV感染的相关因素;年龄40~49周岁(OR=2.12, 95%CI:1.06~4.31)、离异或丧偶(OR=3.70, 95%CI:1.25~10.91)、月收入5000元及以上(OR=0.36, 95%CI:0.17~0.77),以及光顾两种性服务场所(OR=3.22, 95%CI:1.55~6.69)是嫖客人群HSV-2感染的相关因素。结论河口县嫖客人群HIV/STIs感染率较既往下降显著,后续的行为干预工作建议强化安全套使用并重点干预中老年和吸毒人群。Objective To estimate the prevalence and correlated factors of HIV/STIs infection among male clients of female sex workers(FSWs).Methods Cross-sectional survey was conducted in Hekou county,Yunnan province during 2015 to 2016.Information of social-demographic characteristics,sexual and drug using behavior were collected through questionnaire interview.Blood samples were collected to test for HIV,herpes simplex virus-2(HSV-2)and syphilis.Logistic generalized estimating equations model was used to determine the factors associated with HIV/STIs infection.Results The median age of male clients of FSWs was 36 years old.HIV,syphilis,and HSV-2 prevalence among male clients were 2.06%,0.64%,and16.49%.Analyzed with multivariate logistic regression,the factors associated with HIV infection of male clients were age≥50 years(OR=8.11,95%CI:1.26-52.16)and morphine positive(OR=7.35,95%CI:1.42-38.06),age 40~49 years(OR=2.12,95%CI:1.06-4.31),divorced or widowed(OR=3.70,95%CI:1.25-10.91),monthly income>5000 CNY(OR=0.36,95%CI:0.17-0.77),and patronized two brothel based entertainment places(OR=3.22,95%CI:1.55-6.69)were significantly correlated with HSV-2 infection among male clients.Conclusion Compared with past studies,the HIV prevalence decline significantly among male clients of FSWs.Condom use needs to be strengthened in future interventions especially targeting older and drug using male clients of FSWs.
