摘 要:目的探讨慢性阻塞性肺疾病(COPD)患者血清游离三碘甲状腺原氨酸(FT3)、D-二聚体(D-D)、降钙素原(PCT)水平变化及其与肺功能的相关性。方法抽取2017年12月至2019年12月阳泉煤业(集团)有限责任公司总医院收治的COPD患者136例作为病例组,选择同期86例健康体检者作为对照组。比较两组患者以及病例组不同疾病分期患者的血清FT3、D-D、PCT水平,观察血清FT3、D-D、PCT高表达COPD患者及低表达患者改良版英国医学研究会呼吸问卷(mMRC)评分、慢性阻塞性肺疾病自我评估测试问卷(CAT)评分、肺功能指标[第1秒用力呼气容积(FEV1)、用力肺活量(FVC)、FEV1/FVC]水平,以Spearman相关性分析探讨血清FT3、D-D、PCT水平与COPD患者mMRC评分、CAT评分、肺功能指标的关联性。结果病例组血清FT3水平低于对照组,血清D-D、PCT水平高于对照组,且急性加重期患者血清FT3水平低于稳定期患者,血清D-D、PCT水平高于稳定期患者(P<0.05)。血清FT3高表达COPD患者mMRC、CAT评分低于低表达患者,FEV1、FVC、FEV1/FVC水平高于低表达患者(P<0.05);血清D-D、PCT高表达COPD患者mMRC、CAT评分高于低表达患者,FEV1、FVC、FEV1/FVC水平低于低表达患者(P<0.05)。Spearman相关性分析显示,血清FT3表达与COPD患者mMRC评分、CAT评分呈负相关,与FEV1、FVC、FEV1/FVC水平呈正相关,血清D-D、PCT表达与COPD患者mMRC评分、CAT评分呈正相关,与FEV1、FVC、FEV1/FVC水平呈负相关(P<0.05)。结论COPD患者血清FT3水平下降,血清D-D、PCT水平升高,且各指标水平与肺功能、病情程度有关,可为临床诊治提供一定参考。Objective To investigate the levels of serum free triiodothyronine (FT3), D-dimer (D-D), and procalcitonin (PCT) in patients with chronic obstructive pulmonary disease (COPD) and its correlation with pulmonary function.Methods A total of 136 patients with COPD in Yangquan Coal Industry Group General Hospital from December 2017 to December 2019 were selected as the case group, and 86 healthy persons who had physical exam during the same period were selected as the control group. The expressions of FT3, D-D and PCT in serum were determined and compared among patients with different disease stages of the case group, and between the two groups. The modified British Medical Research Council Respiration Questionnaire (mMRC) score, COPD Assessment Test (CAT) score, lung function indexes, including forced expiratory volume in one second (FEV1), forced vital capacity (FVC), FEV1/FVC of COPD patients with high expression and low expression levels of serum FT3, D-D and PCT were observed. Spearman correlation analysis was used to investigate the correlations of serum FT3, D-D and PCT expression with mMRC score, CAT score and pulmonary function indexes in patients with COPD.Results The expression of serum FT3 in the case group was lower than that in the control group, and the expressions of serum D-D and PCT were higher than those in the control group (P<0.05). COPD patients at acute exacerbation stage had lower FT3 level, but higher levels of D-D and PCT, compared to COPD patients at stale stage (P<0.05). COPD patients with high expression of FT3 had lower mMRC and CAT scores, and higher levels of FEV1, FVC and FEV1/FVC, compared to the patients with low FT3 expression (P<0.05). COPD patients with high expression of serum D-D and PCT had higher mMRC and CAT scores, and lower levels of FEV1, FVC and FEV1/FVC, compared to patients with low expression (P<0.05).
