摘 要:目的分析新型冠状病毒肺炎(Corona virus disease 2019,COVID-19)患者不同标本类型核酸检测结果。方法以常州市第三人民医院2020年1月30日-2020年2月23日51例临床诊断为COVID-19患者为研究对象,收集采集的肛拭子及咽拭子标本,采用双重实时荧光定量多聚酶链反应(Dual Quantitative real-time polymerase chain reaction,Dual qPCR)检测新型冠状病毒(SARS-CoV-2),比较两种标本类型病毒核酸检出率。结果51例患者中,咽拭子标本检出率为56.86%,肛拭子标本检出率为21.57%,两种样本检出率差异具有统计学意义(P<0.05)。咽、肛拭子同时检出者与咽拭子检出、肛拭子未检出者ORFlab基因及N基因核酸扩增Ct值比较差异均无统计学意义。观察咽拭子和肛拭子同时检出SARS-CoV-2的患者,发现有7例病例是首先在咽拭子中检出SARS-CoV-2,数天后(6.44±5.68,中位数7天)在肛拭子中检出。结论咽拭子标本与肛拭子标本均可有效检测SARS-CoV-2,两者同时检测可提高SARS-CoV-2病原学诊断率,建议同时采集咽拭子和肛拭子;通过分析咽拭子、肛拭子标本检出时间差,为实验室采集SARS-CoV-2标本时机提供参考。OBJECTIVE To analyze the nucleic acid tests of different types of specimen in patients with Corona Virus Disease 2019(COVID-19).The detection rate was compared to choose more efficient method for early and rapid etiological diagnosis in clinic.METHOD Samples of anal swabs and throat swabs of patients who were diagnosed COVID-19 clinically were collected in the Third People’s Hospital of Changzhou from January 30,2020 to February 23,2020.The novel coronavirus,SARS-CoV-2,was detected by dual quantitative real-time polymerase chain reaction(Dual qPCR).The detection rate of SARS-CoV-2 were compare between anal swabs and throat swabs.RESULTS Twenty-nine cases(56.86%)were detected in throat swab specimens and eleven cases(21.57%)were detected in anal swab specimens among the fifty-one patients.The difference of detection rates between the two specimens was significance(P<0.05).There were no significant differences in the Ct values of ORFlab gene and N gene between both positive throat and anal swab specimens(n=9)and positive throat and negative anal swab specimens(n=20).Seven of specimens in patients,whose throat swab and anal swab were detected positive simultaneously,were found firstly positive in throat swab and then in anal swab several days later(6.44±5.68,median 7 days).CONCLUSION SARS-CoV-2 can be detected effectively in both of the throat swab and anal swab specimens,which can improve the etiological diagnosis rate of novel coronavirus simultaneous collection of the specimens is recommended.Time intervals of positive SARS-CoV-2 detection in throat swabs and anal swabs can give a reference for the proper time to collect specimens of patients.
