摘　　要：目的探讨造血干细胞移植极期患者运用奥瑞姆(Orem)自理模式的临床效果。方法选取我院2014年1月至2019年6月收治的造血干细胞移植极期患者74例为研究对象,随机等分成对照组和研究组,对照组给予常规护理,研究组在对照组基础上通过Orem自理模式进行护理,采用SF-36量表对患者的情感、疼痛、躯体功能、社会、躯体、躯体角色、心理卫生、总体健康进行评分,比较两组患者生活质量以及离院前感染发生率。结果研究组生活质量优于对照组,差异有统计学意义(P<0.05);研究组离院前感染发生率低于对照组,差异具有统计学意义(P<0.05)。结论造血干细胞移植极期患者应用Orem自理模式,可提高患者的生活质量,使患者获得良好就医体验,还可降低医院感染率,值得推广应用。Objective To explore the clinical effect of Orem self-care model during the critical stage of hematopoietic stem cell transplantation.Methods To select 74 cases of patients during the critical stage of hematopoietic stem cell transplantation admitted to our hospital from January 2014 to June 2019 as the study subjects,and they were randomly divided into a control group and a study group.The control group was given routine care,and the study group used Orem self-care model for nursing on the basis of the control group.The SF-36 scale was used to score the patient's emotion,pain,physiological function,social,body,body role,mental health,and general health to compare the quality of life and the incidence of infection before discharge from the hospital between the two groups.Results The quality of life in the study group was better than that in the control group,and the difference was statistically significant(P<0.05).The incidence of infection before discharge in the study group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion The Orem self-care model for patients at the critical stage of hematopoietic stem cell transplantation can improve the patient's quality of life,enable patients to obtain a good medical experience,and reduce the hospital infection rate,which was worth popularizing and applying.
