基于聚类与因子分析研究新安医家王任之治疗月经不调用药规律

摘　　要：目的:使用数据挖掘技术探讨新安医家王任之治疗月经不调的用药规律。方法:从《王任之医案》中选取治疗月经不调的有效处方135首,运用Office 2010、IBM SPSS Modeler 18.0、SPSS Statistics 22.00建立数据库对用药频数、药物组合、用药类别等进行数据挖掘分析。结果:135首处方涉及207味中药,前三类为补血药、活血调经药、补阳药,用药频次达1 769次,频次≥18次共计27味。二项药对使用频次≥26次的药对共32组。由系统聚类分析得到黄芪、甘草、鸡冠花、海螵蛸、五灵脂、香附等9组常用药物组合。由因子分析提取出赤石脂、茜草、地榆、熟地炭、牛膝、泽兰、益母草、卷柏、丹参、柏子仁、熟地黄等20个公因子。结论:王任之辨治月经不调重视肝、肾、气、血的论治,法以养血活血,化瘀为主,并佐疏肝理气、平调阴阳之品,治以补血、化瘀、补阳之剂。Objective: To investigate the medication rule of Xin’an medical practitioner Wang Renzhi in the treatment of irregular menstruation using data mining techniques. Methods: A total of 135 effective prescriptions for the treatment of irregular menstruation were selected from Medical Records of Wang Renzhi, and Office 2010, IBM SPSS Modeler 18.0, and SPSS Statistics 22.0 were used to establish databases and perform a data mining analysis of medication frequency, drug combination, and medication category. Results: These 135 prescriptions involved 207 traditional Chinese medicines, and the top three categories were blood-replenishing medicines, blood circulation-activating and menstruation-regulating medicines, and Yang-tonifying medicines. The medication frequency reached1769 times, and 27 medicines had a frequency of ≥18 times. A total of 32 binomial medicine pairs had a frequency of ≥26 times. The hierarchical cluster analysis obtained 9 combinations of the commonly used medicines including Astragalus membranaceus, Radix Glycyrrhizae, cockscomb, cuttlebone, trogopterus dung, and Rhizoma Cyperi, and the factor analysis obtained 20 common factors including red halloysite, Rubia cordifolia, Sanguisorba officinalis, carbonized prepared Radix Rehmanniae, Achyranthis Bidentatae Radix, Lycopus lucidus, Herba Leonuri, Selaginella tamariscina, Salvia miltiorrhiza, Platycladi seed, and prepared Radix Rehmanniae. Conclusion: In the syndrome differentiation-based treatment of irregular menstruation, Wang Renzhi emphasizes the treatment of the liver, the kidney, Qi, and blood and applies the medicines for soothing the liver, regulating Qi, and balancing Yin and Yang.The blood-replenishing, stasis-resolving, and Yang-tonifying medicines are often used for treatment.
