摘 要:目的采用回顾性研究的方法探讨新疆地区女性人群中阴道微生态的改变是否促进宫颈高危型HPV感染的发生,为本地区高危型HPV感染的防治提供新思路。方法收集2047例在新疆医科大学附属肿瘤医院妇科门诊初次就诊女性人群的阴道感染性疾病和高危型HPV检测结果,统计分析阴道微生态系统与高危型HPV感染的关系。结果635例高危型HPV感染组和1412例未感染组患者之间,白带性状的异常比例、Donders评分、Nugent评分、阴道清洁度、过氧化氢、唾液酸苷酶、白细胞酯酶、β-葡萄糖醛酸苷酶和凝固酶检验结果差异均存在统计学意义(P<0.05)。需氧性阴道炎、细菌性阴道病、阴道微生态双重感染和阴道微生态多重感染均是高危型HPV感染的危险因素,对应的风险值分别是1.428、1.409、1.537和1.643。阴道微生态多重感染或双重感染的情况下,HPV-DNA的负荷量明显高于单纯的AV或者BV感染情况(P<0.05),而阴道微生态多重感染患者的HPV-DNA负荷量也明显高于双重感染患者(P<0.05)。结论新疆地区女性人群像全国其他地区人群一样,阴道微生态的改变会导致高危型HPV感染;同时随着阴道感染复杂性的加深,宫颈感染高危型HPV-DNA的负荷量随之加深;因此,对于阴道微生态异常的人群进行准确诊断和有效治疗有助于降低新疆地区女性人群中高危型HPV感染的发生。Objective To investigate whether it could promote the occurrence of high-risk cervical HPV infection when vaginal microecology changes in Xinjiang women population by a retrospective study,so to provide new thoughts for the prevention and treatment of high-risk cervical HPV infection in this region.Methods The results of vaginal infections and high-risk HPV detection of 2047 women who visited the Gynecology Department of the Tumor Hospital affiliated to Xinjiang Medical University for the first time were collected,and the relationship between vaginal microecosystem and high-risk HPV infection was statistically analyzed.Results Between the 635 high risk HPV infected and 1412 non-infected patients,there were significant differences in the abnormal rate of Leucorrhea,Donders score,Nugent score,vaginal cleanliness,hydrogen peroxide,Sialidase,Leukocyte esterase,β-Glucuronic acid and Coagulase(P<0.05).The risk factors of high risk HPV infection were aerobic vaginitis(AV),bacterial vaginosis(BV),vaginal microecological double infection and multiple infection,and the corresponding RR were 1.428,1.409,1.537 and 1.643,respectively.The load of HPV-DNA in vaginal microecological double infection and multiple infection were significantly higher than those in AV or BV Infection(P<0.05),also the load of HPV-DNA in patients with multiple infection of vaginal microecology was significantly higher than that in patients with double infection(P<0.05).Conclusion In Xinjiang women population,as other regions in China,changes in the vagina microecology can lead to a high-risk HPV infection.Meanwhile,as the complexity of the vaginal infection deepens,the load of high-risk HPV-DNA in the cervix of the uterus increases,so the accurate diagnosis and effective treatment of abnormal vaginal microecology could reduce the incidence of high risk HPV infection in Xinjiang women population.
