以ICSH分级标准为理念的分级报告模式在外周血细胞形态学检验中的效能探究

摘　　要：目的探究基于ICSH分级标准为理念的分级报告模式在外周血细胞形态学检验中的作用。方法选取2018年8月至2019年8月我院检验科已发出报告的488张外周血细胞玻片,其中男性248例(包括住院患者130例,门诊患者118例),女性240例(包括住院患者120例,门诊患者120例)。对比分析传统报告与分级报告对每种形态学异常的检出率,并通过追溯临床诊断分析分级报告的临床意义。结果(1)就红细胞而言,两种报告检出阳性率的差异较为显著,并且两者之间的一致性一般,P<0.01,差异具有统计学意义。就白细胞与血小板而言,两种报告检出阳性率的差异均不显著,但两者之间的一致性较高,因此检出率差异虽不具有统计学意义(P>0.05)但一致性方面的差异具有统计学意义(P<0.01)。(2)红细胞大小不等、小红细胞、低色素性红细胞以及裂片红细胞的分级报告与传统报告之间的检出差异较为显著,P<0.05,差异具有统计学意义。此外红细胞大小不等、小红细胞、低色素性红细胞的一致性均较强,且P<0.01,差异具有统计学意义。就裂片红细胞与椭圆形红细胞而言,虽其一致性一般,但P<0.01,差异具有统计学意义。结论建立外周血细胞形态学分级报告模式有助于对疾病进行鉴别诊断与治疗效果观察。Objective To explore the role of graded reporting model based on the ICSH graded standard in the morphological examination of peripheral blood cells.Methods From August,2018 to August,2019,488 peripheral blood cell hoof films issued by our laboratory were selected for the current study,including 248 males(130 inpatients and 118 outpatients)and 240 females(120 inpatients and 120 outpatients).We compared the detection rate of each morphological abnormality between traditional report and graded report,and analyzed the clinical significance of graded report through the retrospective clinical diagnosis.Results(1)In terms of red blood cells,the difference in the positive rates of two reports was significant,and the consistency between two reports was average(P<0.01).In terms of leukocytes and platelets,the difference in the positive rates of the two reports was not significant,but the consistency between the two was high,suggesting that the difference in the detection rates was not statistically significant(P>0.05)but with a significant consistency(P<0.01).(2)The differences of red blood cells with different sizes,small red blood cells,low pigmented red blood cells,and lobed red blood cells of two reports were significant(P<0.05).In addition,the consistencies of erythrocytes with different sizes,small red blood cells and low pigmented red blood cells were significant between two reports(P<0.01).Regarding lobed red blood cells and oval red blood cells,although their consistency was average,the difference was still statistically significant(P<0.01).Conclusion The establishment of a peripheral blood cell morphology grading report model is helpful for the differential diagnosis and the observation of treatment effects.
