摘　　要：目的分析急性心肌梗死伴糖尿病患者糖原磷酸化酶同工酶BB(glycogen phosphorylase isoenzyme BB,GPBB)及脂蛋白相关磷脂酶A2(lipoprotein-associated phospholipase A2,Lp-PLA2)浓度与心功能的相关性,为临床诊断及治疗提供参考。方法以河北医科大学第一医院2013年1月至2017年12月期间诊治的急性心肌梗死伴糖尿病患者120例为研究对象。以本院同期健康体检者(对照组,n=80)、单纯糖尿病患者(单纯糖尿病组,n=50)、单纯急性心肌梗死患者(单纯急性心肌梗死组,n=70)为对照。比较各组血清中GPBB及Lp-PLA2浓度的差异并分析急性心肌梗死伴糖尿病患者血清中GPBB及Lp-PLA2浓度与心功能的关系。结果与对照组比较,单纯糖尿病组、单纯急性心肌梗死组及急性心肌梗死伴糖尿病组患者血清中GPBB和Lp-PLA2浓度明显升高,差异均有统计学意义(P<0.05),且急性心肌梗死伴糖尿病组患者血清中GPBB和Lp-PLA2浓度最高。随着心功能分级的升高,患者血清GPBB及Lp-PLA2浓度逐步升高,差异有统计学意义(P<0.05)。血清GPBB及Lp-PLA2浓度与心功能分级呈明显的正相关(r=0.458、0.449,P<0.05)。发生心血管事件患者的血清GPBB及Lp-PLA2浓度明显高于未发生心血管事件患者,差异有统计学意义(P<0.05)。结论急性心肌梗死伴糖尿病患者血清中GPBB及Lp-PLA2浓度与心功能呈明显的正相关性。Objectives To analyze the correlations between the serum concentrations of glycogen phosphorylase isoenzyme BB(GPBB)and lipoprotein-associated phospholipase A2(Lp-PLA2)with cardiac function in patients with acute myocardial infarction and diabetes mellitus.Methods Totally 120 patients with acute myocardial infarction and diabetes mellitus treated in The First Hospital of Hebei Medical University from January 2013 to December 2017 were selected as study objects.At the same period,80 physical examinations were selected as control group,50 patients were in diabetes mellitus group and 70 patients were in acute myocardial infarction group.Serum concentrations of GPBB and Lp-PLA2 were compared among the groups and the correlations between serum concentrations of GPBB and Lp-PLA2 with cardiac function in patients with acute myocardial infarction and diabetes mellitus were analyzedResults Compared with control group,serum concentrations of GPBB and Lp-PLA2 in diabetes mellitus group,acute myocardial infarction group,acute myocardial infarction and diabetes mellitus group were significantly higher(P<0.05),and serum concentrations of GPBB and Lp-PLA2 were the highest in acute myocardial infarction and diabetes mellitus group(P<0.05).Serum concentrations of GPBB and Lp-PLA2 positively correlated with cardiac function grading(correlation coefficient r was 0.458 and 0.449 respectively)in patients with acute myocardial infarction and diabetes mellitus.Serum concentrations of GPBB and Lp-PLA2 in patients with cardiovascular events were significantly higher than those in patients without cardiovascular events(P<0.05).Conclusions Serum concentrations of GPBB and LpPLA2 in patients with acute myocardial infarction and diabetes mellitus positively correlate with cardiac function.
