摘　　要：目的探讨股骨近端防旋髓内钉(PFNA)治疗股骨转子间骨折的疗效。方法闭合复位PFNA内固定治疗27例股骨转子间骨折患者。记录手术时间、术中出血量、骨折愈合时间、并发症及外侧壁骨折发生情况,末次随访采用髋关节Harris评分评价疗效。结果手术时间40~102(73.8±2.9)min,术中出血量30~200(80.0±10.1)ml,骨折愈合时间12~17(14.3±2.1)周。患者均获得随访,时间3~24(15.45±7.36)个月。术中发生外侧壁骨折1例,术后发生防旋钉部分退出1例。末次随访髋关节Harris评分91~95(92.7±3.6)分。结论PFNA治疗股骨转子间骨折创伤小,手术时间短,恢复快,术后髋关节功能恢复良好。Objective To investigate the effect of proximal femoral nail antirotation(PFNA)in the treatment of intertrochanteric fracture of femur.Methods The 27 cases of intertrochanteric fracture of femur were treated with closed reduction and PFNA internal fixation.The operation time,bleeding volume during the surgery,the fracture healing time,complications and the occurrence of lateral wall fracture were recorded.Harris score of hip joint was used to evaluate the effect in the last follow-up.Results The operation time was 40~102(73.8±2.9)min,the intraoperative bleeding volume was 30~200(80.0±10.1)ml,and the fracture healing time was 12~17(14.3±2.1)weeks.All patients were followed up for 3~24(15.45±7.36)months.There was 1 case of lateral wall fracture and1 case of partial withdrawal of antirotation nail.In the last follow-up,Harris score was 91~95(92.7±3.6)points.Conclusions In the treatment of intertrochanteric fracture of femur with PFNA,the trauma is less,the operation time is short,the recovery time is short,and the function of hip joint recovers well after operation.
