低温等离子射频治疗变应性鼻炎合并鼻中隔偏曲

摘　　要：目的探讨低温等离子射频治疗变应性鼻炎合并鼻中隔偏曲的临床效果。方法选取2015年3月~2017年12月我院收治的112例变应性鼻炎(AR)合并鼻中隔偏曲的患者为研究对象,按数字随机表法结合动态随机分组将患者分为低温等离子组(L组,63例)和对照组(C组,49例)。L组采用鼻内镜下低温等离子射频治疗,C组采用常规方法手术治疗,并分别于手术后6、12个月随访。记录两组患者术前及术后6、12个月主观症状评分、生活质量问卷(PQLQ)评分、术后复发例数及并发症发生率。测定术前、术后6个月鼻腔黏膜纤毛清除时间及鼻腔阻力。结果两组患者术后6、12个月时喷嚏、鼻涕、鼻塞、鼻痒等主观症状评分及PQLQ评分较术前降低,且术后12个月时,L组较C组显著降低(均P<0.05)。术后12个月时,L组复发率较C组显著下降;L组术后出血、感染及术后6个月鼻腔粘连和下鼻甲萎缩等发生率较C组亦显著下降(均P<0.05)。与术前比较,两组鼻腔黏膜纤毛清除时间缩短,鼻腔阻力降低,且术后6个月时L组鼻腔黏膜纤毛清除时间较C组缩短,鼻腔阻力降低(均P<0.05)。结论采用低温等离子射频治疗伴鼻中隔偏曲的变应性鼻炎,可以改善患者喷嚏、鼻涕、鼻塞和鼻痒等主观症状,降低患者的复发率,缩短鼻腔黏膜纤毛清除时间,降低鼻腔阻力。Objective To evaluate the clinical effect of low temperature plasma radiofrequency on allergic rhinitis with deviation of nasal septum.Methods From March 2015 to December 2017,112 patients with allergic rhinitis(AR)with deviation of nasal septum were treated and followed up in Sichuan Bayi Rehabilitation Center.The patients were divided into low temperature plasma group(group L,63 cases)and control group(group C,49 cases)using a random number table method and dynamic random grouping.Group L was treated with low-temperature plasma radiofrequency under nasal endoscopy,while group C was treated with routine surgery.The patients were followed up 6 months and 12 months after operation.The subjective symptoms scores,PQLQ scores,the number of recurrences and complications were recorded before and 6 and 12 months after operation,and the curative effect were evaluated.Nasal mucociliary clearance time and nasal resistance were measured before and 6 months after operation.Results Compared with group C,the scores of subjective symptoms such as sneezing,runny nose,nasal obstruction and nasal itching and PQLQ scores in group L was decreased at 12 months after operation(P<0.05).Compared with those before operation,the scores of subjective symptoms such as sneezing,runny nose,nasal obstruction and nasal itching and PQLQ scores in group L was decreased at 6 and 12 months after operation(P<0.01 or<0.05).The recurrence rate and complications at 12 months after operation in group L was significantly lower than that in group C(P<0.01).Compared with group C,nasal mucociliary clearance time was shortened and nasal resistance was decreased in group L at 6 months after operation(P<0.05 or<0.01).Compared with pre-operation,nasal mucociliary clearance time was shortened and nasal resistance was decreased in both groups(P<0.01).Conclusion The treatment of allergic rhinitis with deviation of nasal septum by low temperature plasma radiofrequency can improve subjective symptoms such as sneezing,runny nose,nasal obstruction and nasal itching,re
