六君子汤对肺脾气虚型自发性气胸的疗效分析

摘　　要：目的探讨与分析六君子汤对肺脾气虚型自发性气胸的疗效与机制。方法肺脾气虚型自发性气胸患者112例根据随机数字表法把患者分为研究组与对照组各56例。所有患者都给予胸腔闭式引流术治疗,对照组术后接受常规治疗,研究组在对照组治疗的基础上给予六君子汤治疗,记录两组术后恢复情况与血清转化生长因子β受体1(TGF-βreceptor 1,TβR1)与白介素-18(Interleukin-18,IL-18)表达变化情况。结果术后14 d研究组的总有效率为98.2%,显著高于对照组的87.5%(P<0.05)。两组手术时间对比差异无统计学意义(P>0.05),研究组术后住院时间、术后胸腔积气缓解时间等显著少于对照组(P<0.05)。研究组术后14 d的感染、肺不张、皮下气肿等发生率为3.6%,显著低于对照组的17.9%(P<0.05)。两组术后14 d的TβR1、IL-18值都显著低于术后1 d(P<0.05),研究组也显著低于对照组(P<0.05)。结论六君子汤在肺脾气虚型自发性气胸术后患者的应用能促进患者康复,抑制炎症因子的释放,减少术后并发症,从而提高治疗效果。Objective To investigate and analysis the efficacy of Liujunzi Decoction on spontaneous pneumothorax with lung spleen qi deficiency and to explain the related mechanisms.Methods 112 patients with spontaneous pneumothorax are equally divided into study group and control group accorded to the random number table method.All patients are treated with thoracic closed drainage,and the control group are received routine treatment,the study group are treated with Liujunzi decoction on the basis of the control group.The postoperative recovery are recorded,and the expression changes of growth factor beta receptor 1(TGF-βreceptor 1,TβR1)and interleukin-18(IL-18)are detected.Results There are no significant difference in the operation time compared between the two groups(P>0.05).The postoperative pleural effusion remission time and postoperative hospital stay in the study group are significantly less than in the control group(P<0.05).The postoperative 14 d of the total effective rates of the study group are 98.2%,which are significantly higher than 87.5%of the control group(P<0.05).The postoperative 14 d of the incidences of subcutaneous emphysema,atelectasis,and infection in the study group are 3.6%,which are significantly lower than that of the control group(17.9%)(P<0.05).The postoperative 14 d of the serum TβR1 and IL-18 values of the two groups are significantly lower than those of the postoperative 1d(P<0.05),and the study group are also significantly lower than the control group(P<0.05).Conclusion The application of Liujunzi Decoction in patients with spontaneous pneumothorax with lung spleen qi deficiency can promote the rehabilitation of patients,improve the therapeutic effect,inhibit the release of inflammatory factors,and reduce the occurrence of postoperative complications.
