摘 要:目的探讨和分析乌司他丁对老年急性呼吸窘迫综合征患者炎性因子及GSH-PX、SOD等氧化因子的影响。方法选取郑州大学第五附属医院于2018年3月—2019年10月期间收治急性呼吸窘迫综合征老年患者80例,按照交叉双盲法分为两组,对照组40例患者给予常规对症治疗,观察组40例患者在此基础上联合乌司他丁治疗。根据IL-6、IL-10、TNF-α等水平,对比两组治疗前后炎性因子水平。根据PVPI、EVLWI等指数水平,对比两组治疗前后肺微循环阻力指标。根据GSH-PX、SOD水平,对比两组治疗前后氧化应激状态。根据后PaO_2、PaO_2/FiO_2等水平,对比两组治疗前后相关血气指标。结果治疗前两组IL-6、IL-10、TNF-α等水平无差异(P>0.05),治疗后观察组各项炎性因子水平低于对照组,组间有差异(P<0.05)。治疗前两组PVPI、EVLWI等指数水平无差异(P>0.05),治疗后观察组PVPI、EVLWI水平优于对照组,组间有差异(P<0.05)。治疗前两组GSH-PX、SOD等水平无差异(P>0.05),治疗后观察组GSH-PX、SOD水平优于对照组,组间有差异(P<0.05)。治疗前两组PaO_2、PaO_2/FiO_2等水平无差异(P>0.05),治疗后观察组PaO_2、PaO_2/FiO_2水平优于对照组,组间有差异(P<0.05)。结论为进一步提高临床疗效,本次研究对老年急性呼吸窘迫综合征患者给予乌司他丁治疗,可缓解机体氧化应激、炎性应激反应,降低肺微循环阻力,同时症状改善效果更显著,提高血气指标水平,值得推广。Objective To investigate and analyze the effect of ulinastatin on inflammatory factors,GSH-Px,SOD and other oxidation factors in elderly patients with acute respiratory distress syndrome.Methods Totally 80 elderly patients with acute respiratory distress syndrome were enrolled in our hospital from March 2018 to October 2019.According to the cross double blind method,they were divided into two groups.40 patients in the control group were given routine symptomatic treatment,and 40 patients in the observation group were treated with ulinastatin on this basis.According to the levels of IL-6,IL-10 and TNF-α,the levels of inflammatory factors in the two groups before and after treatment were compared.According to the indexes of PVpi and EVLWI,the indexes of pulmonary microcirculation resistance before and after treatment were compared between the two groups.According to GSH-Px and SOD levels,the oxidative stress state of the two groups before and after treatment was compared.According to the levels of PaO2 and PaO2/FiO2,the blood gas indexes of the two groups before and after treatment were compared.Results There was no difference in the levels of IL-6,IL-10 and TNF-αbetween the two groups before treatment(P>0.05).After treatment,the levels of inflammatory factors in the observation group were lower than those in the control group,and there was a difference between the two groups(P<0.05).There was no difference in PVPI and EVLWI between the two groups before treatment(P>0.05).After treatment,the level of PVpi and EVLWI in the observation group was better than that in the control group,and there was difference between the two groups(P<0.05).There was no difference in GSH-Px and SOD levels between the two groups before treatment(P>0.05).After treatment,GSH-Px and SOD levels in the observation group were better than those in the control group,and there was difference between the two groups(P<0.05).There was no difference in PaO2 and PaO2/FiO2 levels between the two groups before treatment(P>0.05).After treatment,the PaO
