初治慢性HBV感染者血清标志物定量与血常规、肝功能及病毒载量相关性分析

摘　　要：目的分析初治慢性HBV感染者血清标志物(HBsAg、HBcAb)定量水平与其他指标的相关性,研究血清标志物的临床意义。方法717例初治慢性HBV感染者根据HBeAg状态分为2组:HBeAg阳性与HBeAg阴性组。对2组患者的血常规、肝功能、HBV DNA、HBsAg定量、HBcAb定量等进行检测比较。结果HBeAg阳性慢性HBV感染者血清HBsAg水平与HBV DNA呈中等相关(r=0.42,P<0.001);血清HBcAb水平与ALT(r=0.36,P<0.001)、AST(r=0.37,P<0.001)呈中等相关,当ALT>120 U/L,HBcAb水平不再随之升高。HBeAg阴性慢性HBV感染者血清HBsAg水平与HBV DNA(r=0.14,P=0.08)仅弱相关。HBeAg阴性者血清HBcAb水平ALT(r=0.37,P=0.01)、AST(r=0.39,P<0.001)呈中等相关,ALT高水平者(>120 U/L)HBcAb定量显著高于ALT低水平者(≤120 U/L)。结论初治HBeAg阳性慢性HBV感染者血清HBsAg水平与HBV DNA呈中等正相关,HBeAg阴性者血清HBsAg水平与HBV DNA呈弱相关。HBeAg阳性与阴性慢性HBV感染者血清HBcAb水平与ALT均呈正相关。Objective To investigate the correlation between the quantitation of serum markers levels(HbsAg and HbcAb)and the other indexes in initial treatment of chronic HBV infection,and to explore the clinical significance of serum markers.Methods A total of 717 patients with chronic HBV infection were enrolled in the study,who were divided into HBeAg positive group and HBeAg negative group.The blood routine test,liver function,and the levels of ALT,HBV DNA,HBsAg,HBcAb were detected and compared between the two groups.Results The serum HBsAg levels in chronic HBV infection patient with positive HbeAg were moderately correlated with those of HBV DNA(P<0.01),and serum HBcAb levels were moderately correlated with those of ALT and AST(P<0.01).When ALT levels were above 120U/L,the serum HBcAb levels were not increased along with ALT.Moreover the serum HBsAg levels in chronic HBV infection patient with negative HbeAg were weakly correlated with those of HBV DNA(P>0.05),and the serum levels of HBcAb were moderately correlated with those of ALT and AST(P<0.01).In addition the HbcAb levels in patients with high ALT levels>120U/L were significant higher than those in patients with low ALT levels≤120U/L.Conclusion The serum HBsAg levels in chronic HBV infection patient with positive HbeAg are moderately correlated with those of HBV DNA,and the serum HBsAg levels in chronic HBV infection patient with negative HbeAg are weakly correlated with those of HBV DNA,moreover,the serum HBcAb levels in chronic HBV infection patient with positive HbeAg and the patients with negative HbeAg are positively correlated with those of ALT.
