摘  要:目的观察慢性阻塞性肺疾病急性加重期患者血清过氧化脂质(LPO)、谷胱甘肽过氧化物酶(GSH-Px)、过氧化氢(CAT)水平,探讨其氧化应激水平与肺功能的相关性,为慢阻肺患者的多方面治疗提供指导思路。方法选择2014年6月至2016年6月符合标准的慢阻肺急性加重期患者60例(慢阻肺组)和健康体检者60例(对照组),检测血清LPO、GSH-Px、CAT水平,同时测定肺功能,比较健康体检者和慢阻肺急性加重期患者的氧化应激水平和不同分级肺功能的慢阻肺患者的氧化应激水平,分析其与肺功能的相关性。结果慢阻肺组患者血清LPO水平明显高于对照组,而GSH-Px、CAT水平明显低于对照组,差异有统计学意义(P<0.01)。慢阻肺患者血清LPO水平随着肺功能受损程度的加重而数值逐渐升高,其与肺功能呈正相关(P<0.05),GSH-Px、CAT水平随着肺功能受损程度的加重而数值逐渐降低,其与肺功能呈负相关(P<0.05)。结论氧化/应激是慢阻肺功能受损的原因之一,临床上可根据以上资料对患者病情及预后进行初步判断并进行相关治疗,以最大程度减轻患者痛苦及降低经济负担,从而为临床服务。Objective To investigate the levels of lipid peroxide(LPO),glutathione peroxidase(GSH-Px),hydrogen peroxide(CAT)in patients with acute exacerbation of chronic obstructive pulmonary disease(COPD),and to explore the correlation between oxidative stress and lung function of patients,so as to provide guidance idea for various treatments for patients with COPD.Methods A total of 60 patients with COPD at acute exacerbation period who were treated in our hospital from June 2014 to June 2016 were enrolled as COPD group,at the same time,the other 60 subjects who underwent physical examination in our hospital were enrolled as control group.The serum levels of LPO,GSH-Px,CAT and lung function were measured,moreover the oxidative stress levels were observed and compared between the two groups.In addition the correlation between the oxidative stress levels and lung function was analyzed.Results The serum levels of LPO in COPD group were significantly higher than those in control group,however,the levels of GSH-Px and CAT in COPD group were significantly lower than those in control group(P<0.01).The serum levels of LPO in COPD group were increased with the increase of the damage degree of lung function,which were positively with lung function(P<0.05).The serum levels of GSH-Px,CAT in COPD group were decreased with the increase of damage degree of lung function,which were negatively correlated with lung function(P<0.05).Conclusion The serum levels of LPO are increased in patients with COPD at acute exacerbation period,which are positively correlated with lung function,and the levels of GSH-Px and CAT are decreased,which are negatively correlated with lung function.Moreover oxidative stress is one of the causes of pulmonary function damage of patients with COPD,clinically,the disease condition and prognosis of patients can be preliminarily judged and relevant treatment can be conducted according to the above data,in order to minimize the pain of patients and reduce the economic burden.
