摘 要:目的:探讨弹力压迫硅胶喷剂外用联合强脉冲光治疗烧伤后瘢痕增生的临床效果及安全性。方法:烧伤后瘢痕增生患者84例(651处烧伤),采用随机数表法分为观察组和对照组各42例。对照组给予弹力压迫硅胶喷剂外用治疗,观察组在对照组基础上加施强脉冲光治疗,均持续治疗4个月。于治疗结束后评估两组临床疗效,比较两组治疗前后瘢痕厚度、胶原纤维面密度、温哥华瘢痕量表(VSS)和中文版精简烧伤健康量表(BSHS-B)评分,记录两组治疗期间不良反应发生情况。结果:治疗后观察组的总有效率为90.06%,明显高于对照组的84.95%,两组间比较差异有统计学意义(P<0.05);治疗后两组患者的瘢痕厚度、胶原纤维面密度、VSS评分均较治疗前减小,BSHS-B评分较治疗前上升,两组间比较差异有统计学意义(P<0.05);观察组治疗后的瘢痕厚度、胶原纤维面密度、VSS评分明显小于对照组治疗后,BSHS-B评分明显大于对照组治疗后,两组间比较差异有统计学意义(P<0.05);两组不良反应总发生率比较差异无统计学意义(P>0.05)。结论:弹力压迫硅胶喷剂外用联合强脉冲光治疗烧伤后瘢痕增生临床效果确切且安全性较高。Objective:To analyze the clinical effect and safety of elastic compression silicone spray for external application combined with intense pulsed light(IPL)in the treatment of scar hyperplasia after burn.Methods:84 patients(burns at 651 sites)with scar hyperplasia after burn were divided into observation group and control group according to random number table method,42 cases in each group.The control group was treated with elastic compression silicone spray for external application,while the observation group was additionally treated with IPL.All were continuously treated for 4 months.After treatment,the clinical curative effect of both groups was evaluated.The scar thickness,density of collagen fiber surface,scores of Vancouver scar scale(VSS)and burn specific health scale-brief(BSHS-B)before and after treatment were compared between the two groups.The occurrence of adverse reactions during treatment in both groups was recorded.Results:After treatment,the total effective rate of the observation group was significantly higher than that of the control group(90.06%vs.84.95%)(P<0.05);the scar thickness,density of collagen fiber surface and VSS scores of both groups were significantly decreased,while BSHS-B scores were significantly increased(P<0.05);the scar thickness,density of collagen fiber surface and VSS scores in the observation group were significantly less than those in the control group,while BSHS-B scores were significantly higher than those in the control group(P<0.05);there was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion:The clinical curative effect of elastic compression silicone spray for external application combined with IPL is effective and safe in the treatment of scar hyperplasia after burn.
